About this file

This document is a Vendor Performance Report from the Arizona Department of Administration's State Procurement Office, dated 9/30/22. The report involves the Arizona Department of Juvenile Corrections (ADJC) and Avertest LLC - Averhealth, with the specific contact being Courtney Clements. The report was initiated by Kerry Wells and pertains to a performance issue with a drug testing contract.

The contract (CTR051336) is for drug testing services for youth, specifically at the Adobe Mountain School and Community Corrections program. The report highlights a critical performance issue where 13 drug tests initially showed positive results for fentanyl - 10 from juveniles at the Adobe Mountain School and 3 from the Community Corrections program. Upon confirmation testing, ADJC discovered that all 13 positive results were false, prompting a request for a written corrective action plan from the vendor. The procurement action involved sending the report to Courtney Clements to address these test result inaccuracies and develop a detailed corrective plan to resolve the testing disruptions.
