About this file

The document is a Stop Work Order issued by the Arizona Department of Juvenile Corrections (ADJC) to Avertest LLC - Averhealth, located in Richmond, VA, for Contract CTR051336 related to drug testing services for youth. The Stop Work Order was issued on September 30, 2022, and addresses non-compliance with contract quality standards and terms.

The specific contract requires drug testing services, and the Stop Work Order mandates that Avertest LLC immediately cease providing single fentanyl oral tests and 10-panel tests that include fentanyl, reverting instead to the previously used 9-panel test. The order will remain in effect until ADJC reviews and approves a written corrective action plan requested in a Vendor Performance Report (VPR). ADJC reserves the right to take further contract action, potentially including termination for default, if Avertest does not submit an acceptable corrective action plan by the specified date.
