About this file

This is a vendor affidavit form required by Hillsborough County Public Schools (HCPS) in Florida, mandated under Section 787.06(13) of the Florida Statutes regarding human trafficking and labor coercion. The affidavit must be completed and signed by an authorized officer or representative of any nongovernmental contractor executing, renewing, or extending a contract with HCPS. The document applies to the Fire Protection System Services contract opportunity, for which HCPS Communications and Electronics is seeking certified firms to provide inspection, service, and repair of pre-engineered and engineered CO2 automatic fire extinguisher systems and sprinkler systems across all HCPS sites. The contractor must implement the District's procedures and policies for fire protection system provision, with a structural requirement that one vendor perform inspections while a different vendor performs repairs to eliminate conflicts of interest.

By executing this affidavit under penalty of perjury, the contractor certifies that the company does not use coercion for labor or services, including but not limited to physical force, unlawful restraint or confinement, debt bondage linked to labor, confiscation of identification documents, threats of financial harm, fraud or deceit in recruitment, or provision of controlled substances for exploitation. This affidavit is a mandatory compliance document that must accompany the contractor's proposal and serves as a prerequisite for contract award. The affidavit requires completion of signature, name, title, and date fields by the authorized company representative.

• destroy, conceal, remove, confiscate, withhold, or possess any actual or purported passport, visa, or other immigration document, or any other actual or purported government identification document, of any person; or
