About this file
This is an instruction document from the City of Philadelphia related to uploading a Consent and Authorization Form to the PHLContracts vendor profile system for the ADA Mentor Protege Pilot Program in Pennsylvania. The document provides step-by-step guidance for vendors to submit required documentation through the online procurement portal. Vendors must log into PHLContracts using the Seller Administrator role, navigate to Maintain Organization Information, access Maintain General Organization Information, and upload the required form through the Attachments section. The ADA Mentor Protege Pilot Program is estimated between $1,000,000.00 and $2,000,000.00 with an associated bid processing fee of $200.00.
The form submission process requires vendors to upload only the last two pages of the Consent and Authorization Form, as the first four pages contain instructional material and are not part of the required submission. This documentation requirement is a standard prerequisite for vendor participation in the City of Philadelphia's procurement process and must be completed to establish or maintain an active vendor profile in the system.
