TECHNICAL ACCEPTABILITY QUESTIONNAIRE

Please mark the appropriate response to each question as it relates to your ability to meet the terms and conditions of the solicitation and requirements of the Statement of Work. All references are to a specific section of the solicitation, or paragraph of the Statement of Work:

1. Contractor agrees, it will not pay more than 50% of the amount paid by the government to it to firms that are not VIP-listed SDVOSBs.

2. Contractor shall ensure all handling, licensing, transporting and final disposal is in accordance with all ADH, DOT, ADEQ, FDA, NIOSH, federal, state and local regulations and codes.

20.H

3. Contractor will ensure all transportation vehicles shall be equipped with fully enclosed leak-resistant cargo bodies maintained in a sanitary condition and secured when left unattended.

4. Vehicles shall bear the transporter's name, and State permit 20.F

5. Contractor shall provide adequate supervision with technical and administrative abilities needed to meet contract requirements.

6. Contractor shall maintain a log of regulated solid waste and Recycled Waste which include quantity of waste shipped, sources, and delivery points.

7. Contractor shall provide their employees with appropriate protective clothing, equipment, and apparel as prescribed by the Occupational Safety & Health Administration (OSHA).

8. The contractor shall provide CAVHS monthly totals of all items processed for each facility. (e.g. total volumes of solid waste and recycled waste).

9. The Contractor shall ensure that the waste is secured at all times to 20.A protect and maintain the confidentiality of information of patients, employees and the facility.
