About this file
This is a Sole Source Letter dated May 1, 2025, provided to Daniel Prokop, the SORH Manager at the Ohio State Office of Rural Health, concerning the Ohio Critical Access Hospital (CAH) Swing Bed Outcomes Project. The document involves multiple organizations, including Stroudwater Associates, the University of Minnesota's Rural Health Care Research Center (UMRHRC), and the Federal Office of Rural Health Policy, with a focus on a statewide extension of a national CAH Swing Bed quality measurement project.
The document describes a unique, HIPAA-compliant data collection and benchmarking system specifically designed for Critical Access Hospitals' Swing Bed services. Stroudwater Associates is positioned as the sole provider of this specialized service, with their program currently encompassing over 250 CAHs across 21 states as of June 2024. The project offers a proprietary, risk-adjusted comparative analytics platform for CAH Swing Bed outcomes, leveraging exclusive research, technical infrastructure, and partnerships with UMRHRC and the Rural Hospital Flexibility (Flex) grant program. The sole source justification is based on the absence of an equivalent integrated CAH Swing Bed outcomes data collection, reporting, and benchmarking option for Ohio CAH providers.

SOLE SOURCE LETTER
for Ohio Critical Access Hospital Swing Bed Outcomes Project
PROVIDED TO: Daniel Prokop, SORH Manager
Ohio State Office of Rural Health
DATE: May 1, 2025
The proposed Ohio Critical Access Hospital (CAH) Swing Bed Care Outcomes renewal project serves as a CAH-endorsed statewide extension of the national CAH Swing Bed quality measurement project developed and supported by the University of Minnesota’s
Rural Health Care Research Center (UMRHRC). The proposed Ohio Swing Bed
Outcomes project leverages the unique findings, processes and technical infrastructure that is available only through the RHRC study funded by the Federal Office of Rural
Health Policy.
Background. CAHs receive cost-based reimbursement for acute and post-acute (i.e., Swing Bed) healthcare services, unlike their Prospective Payment System (PPS) counterparts. As a result, CAHs are unique healthcare providers and therefore technical assistance, advisory services and performance improvement initiatives have to be tailored to their unique delivery models and payment systems. There are few professional services or data analytics firms that have developed tools and products that fit with the specific requirements of CAHs. Stroudwater Associates has developed a unique CAH-specific set of quality improvement tools and a dedicated website to address the needs of CAHs and has developed these resources in exclusive partnership with the UMRHRC and the Rural
Hospital Flexibility (Flex) grant program. The sole source selection is justified because there is no equivalent program nor opportunity that enables Ohio CAHs to participate in an industry-leading, CAH-specific data collection, reporting and national benchmarking system. Stroudwater’s program include over 250 CAHs representing 21 states as of June
2024. As a result, there is no comparable integrated CAH Swing Bed outcomes data collection, reporting and benchmarking option available to the Ohio CAH Swing Bed providers.
Differentiation. Stroudwater Associates maintains the only national performance improvement, data collection and benchmarking system for CAH Swing Bed outcomes measures in the United States. The Stroudwater program, in partnership with UMRHRC, is a secure, HIPAA-compliant web-based platform that provides proprietary, risk-adjusted comparative analytics of CAH Swing Bed data. As a vendor, Stroudwater is the only organization that can offer the Ohio CAHs the necessary infrastructure, data architecture, research basis, installed base and experience to develop a consistent data collection, reporting and benchmarking model.
