About this file

This is a supplement form to AFLC Form 761/762 that outlines qualification requirements for manufacturers, with sections addressing requirements both prior to and after October 19, 1984. The form establishes specific criteria for qualifying as an approved source for manufacturing certain items.

The form contains three main sections (F.1, F.2, and F.3) for each time period, with detailed requirements for manufacturer qualification. Key provisions include that prequalification is essential for maintaining quality and operational safety, and manufacturers must complete testing requirements set forth in MQR-848 for 761s or RQR-848 for 762s. The form provides options for qualifying either through direct testing by the original design company or through testing on similar items determined by the government to be comparable in complexity and criticality. For requirements after October 19, 1984, a Justification for Qualification Requirement (JQR) is required, and additional provisions under FAR 9.206-1 and AFMCFARS apply.
