Quality Assurance Surveillance Plan (QASP)

I. Objective: The purpose of this plan is to provide a quality assurance surveillance plan for use by the Contracting Officer’s Representative (COR) in support of the technical and managerial support being provided to the Government. This plan provides a basis for the COR to evaluate the quality of the Contractor's performance.

II. Performance Standards:

A. Quality Level: By monitoring the contractor, the COR will determine whether the contractor is performing up to industry standards and expectations and up to the requirements of the Contract.

B. Frequency: During performance of this Contract, the COR will make periodic observations (i.e., conduct surveillance) of the contractor’s performance.

C. Management Responsiveness: The COR will determine whether the contractor has managed the Contract effectively and efficiently, with successful and timely response to user and client needs. The COR will confirm whether the contractor has satisfactorily met all reporting requirements, including subcontracting reports when applicable.

III. Evaluation Methods: The COR will conduct performance evaluations based upon Section II above and the required performance levels set forth in the contract. The following techniques will be used to perform surveillance on all 4 work streams, as follows:

TASK ONE – Tank Management Plan (TMP) TASK TWO - Maintain/Enhance Existing SRM Project Tracker TASK THREE – Develop System Change Request Reports TASK FOUR - Update and Finalize Existing Process Maps and Standard Operating Procedures (SOPs)

A. Performance Monitoring. The Contractor is required to provide sufficient qualified personnel to perform the requirements of the Contract. Customer feedback will be sought. Further, the COR will review measures taken by the Contractor to keep all customers informed of situations that may affect performance and schedules. Notifications of problems encountered will be duly noted by the COR and provided to the Contractor on a monthly basis.

B. Interviews with Assigned Performance Monitors. Quarterly, the COR will discuss the performance of the Contractor on the specified tasks. The COR will note any problems or positive accomplishments and document them.

C. Consultation with the Contractor. The COR will periodically, but no less than quarterly, meet with the Contractor’s Project Manager and the contractor’s Task Leads to discuss any problems, identify circumstances beyond control of the Contractor, identify possible remedies, and note positive accomplishments. The COR will document the meeting.

IV. Issue Resolution: Any issues not resolved in consultations will be communicated to the Contracting Officer for resolution.
