About this file
This is a Scope of Work document for a Request for Quotes (RFQ) issued by the Indiana State Psychiatric Hospital Network (ISPHN) for regulated medical waste (RMW) removal services across three facilities: Madison State Hospital, Logansport State Hospital, and Richmond State Hospital. The RFQ seeks professional waste collection, transportation, and disposal services compliant with federal, state, and local regulations, including standards set by The Joint Commission, Centers for Medicare and Medicaid Services, Environmental Protection Agency, and Department of Transportation. Services include supplying compliant packaging materials (red bags and sharps containers), maintaining consistent pickup schedules with emergency response capability within 24 hours, and providing DOT Hazardous Materials Training. Madison State Hospital requires one pickup every 12 weeks (estimated 35 containers annually, 32-gallon containers); Logansport State Hospital requires one monthly pickup (estimated 14 containers annually, 28-gallon containers); and Richmond State Hospital requires four monthly pickups (estimated 96 containers annually, 28-gallon containers). All pickups are scheduled between 8:00 a.m. and 3:00 p.m. Eastern Time, excluding state holidays. Bidders may submit quotes for one or more facilities, with preference potentially given to multi-facility proposals.
Performance requirements mandate on-time completion of all scheduled pickups, accurate documentation including waste manifests and certificates of destruction, safe handling with no contamination or spills, and response to inquiries within one business day. The contractor must maintain proper container labeling with biohazard symbols and contractor information, allow 24-hour cancellation notice with no charge, and dispose of waste at licensed treatment or incineration facilities. Monthly service reporting is required, detailing service dates, waste quantities removed, disposal methods and facilities used, any incidents or corrective actions, and itemized spend comparisons by facility. The document does not specify response deadlines, award dates, contract term, or pricing information.
