1 SCOPE
Project. The requested work occurs in Building 380 on Eglin Air Force Base. Proposal to replace known defective and missing parts from Chiller 3 VSD. Project includes
Install a new power assembly, logic board, trigger board, and bus isolation board.
Replace the VSD coolant tubing on the back of the drive and replenish the variable speed drive with new inhibitor fluid.
Install a clean up kit on the chiller due to being shut down and open to atmosphere for a long period of time.
Does not include replacing the shaft seal, re-installing the oil filter housing, vacuuming the machine or recharging with refrigerant.
The drive has not been powered and parts quoted are known parts that require replacing, any additional issues are not included.
In-line duct and piping devices, including, but not limited to valves, dampers, humidifiers, wells, taps, flow meters, orifices, etc., if required to be furnished
Exclusions:
1.Labor or material not specifically described above is excluded from this proposal.
2.Unless otherwise stated, any and all overtime labor is excluded from this proposal.
3.Applicable taxes or special freight charges are excluded from this proposal
The Contractor shall be required to furnish all materials, equipment, labor and personnel necessary to accomplish this task order in accordance with the contract requirements. The Contractor is expected to maintain, at all times, good and accepted construction and safety standards.
Title and Location. Project Replace Parts for Chiller 3 VSD, Building 380; Eglin AFB, Florida.
The Project areas are located at the corner of Eglin Blvd, and Sixth St. Contact the Government contact to coordinate access to the building and spaces. Briefings will follow initial contact for information re. Deliveries and lay-down areas.
2. EXISTING CONDITIONS:
Chiller 3 located at Building 380 is currently inoperable.
3. TYPICAL FOR PROJECT AREA:
The work shall be designed to comply with all applicable Federal, State and local requirements and other standards referenced herein. The Florida Department of
Environmental Protection regulations referenced in this SOW are considered as contract documents; thus, shall be applied to all work performed under this contract. The work includes but is not limited to the following:
a) General Scope: Repair of Chiller at Bldg 380 project located on Eglin AFB.
b) Permits: If required, prepare and obtain all environmental permitting including:
• Eglin Air Force Base permits, AF 103 dig permit, etc.
• The Contractor shall pay all permits fees.
c) Salient Characteristics: See Sections 2 (Existing Conditions) and 3 (Figures)
d) Site Visit: NA
The Contracting Officer will schedule the date and time for delivery of equipment.
Any concerns or questions shall be in writing, directed to the Contracting Officer.
e) Base Access: Any person, individual, entity, or company requiring base access shall contact the Contracting Officer to obtain and process an access permit. Current Photo Identification, Current Driver’s License, Current Registration, and Current Vehicle Insurance shall be required for vehicle access pass. Allow up to 10 days for Base access. Base Contracting can submit completed Base Access Request for up to six-months. Commercial vehicles shall use the Contractor’s Gate and are subject to search. The entire contractor workforce shall be U.S. citizens.
f) Phasing: NA
g) Performance Time: Delivery
The Period of Performance will be 75 days. Any work performed on federal holidays must be coordinated with Contracting Officer. Work shall be performed during the normal work hours: 0700 – 1700.
Due to lead times on equipment being 60 days, the POP is 75 days.
h) Permits: The Contractor shall be responsible for obtaining the permits required to perform this project including digging permits, storm water, burning permits, and NPDES permits.

4. MISHAP NOTIFICATION REQUIREMENTS for CONTRACTORS PER AFI 91-204, para 2.4.5: The contractor shall notify the 96TW Safety Office telephonically within one (1) hour after initial notification of all mishaps or incidents involving damage to DoD property
(material plus labor) or injuries to government personnel as a result of contractor operations and work-related injury to contractor personnel that results in fatality, in-patient hospitalization, amputation, or eye loss. The safety Office can be reached at (850)-882-2540 extension 3 during duty hours and (850)-883-4020 after hours. Initial written confirmation of the mishap must be forwarded within three calendar days to the Contracting Officer (CO) who will forward the report to 96 TW/SEG (Safety Office). A complete report shall be provided within 20 calendar days after the mishap. Mishap notification shall contain, as a minimum, the following information:
a) Contract, Contract Number, Name and Title of Person(s) Reporting
b) Date, Time and exact location of accident/incident
c) Brief Narrative of accident/incident (Events leading to accident/incident)
d) Cause of accident/incident, if known
e) Estimated cost of accident/incident (material and labor to repair/replace)
f) Nomenclature of equipment and personnel involved in accident/incident
g) Corrective actions (taken or proposed)
The contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records until released by the 96TW Safety Office. The contractor and subcontractors (if involved) shall cooperate fully and assist the government personnel during the investigation.
