About this file
This document is an Affirmation and Disclosure Form from Ohio Brush Control LLC, located in Alexandria, Ohio, related to a state contract with the Ohio Department of Transportation (ODOT). The form addresses compliance with Executive Order 2022-02D, which prohibits purchasing services from Russian companies or institutions, and includes provisions for performing services and handling state data exclusively within the United States.
The document does not specify detailed contract goods or services, but it requires the contractor to affirm that all services and data management will be performed within the United States. The contractor must disclose any changes in service locations before, during, and after contract execution, with ODOT retaining the right to terminate the contract if services are moved outside the United States without a signed waiver. The form was executed by Harrison Duffey, the owner of Ohio Brush Control LLC, on 10/08/24, and serves as a compliance document for potential state contract engagement.
