About this file

This is a Service Provider Security Assessment Questionnaire required for a South Carolina Department of Mental Health (SCDMH) Vehicle Management Software contract opportunity. The questionnaire is a mandatory compliance document that bidders must complete to demonstrate their information security capabilities and practices. Prospective contractors must provide detailed responses addressing eleven core security areas, including access control policies, disaster recovery planning, employee and contractor vetting procedures, sub-contractor management, third-party security certifications, physical security measures, encryption protocols, breach prevention safeguards, incident response procedures, and data management practices upon contract termination.

The questionnaire requires bidders to submit comprehensive documentation demonstrating that adequate security controls exist to protect the confidentiality, integrity, and availability of government information systems. Acceptable evidence includes third-party certifications such as ISO/IEC 27001 compliance certificates, AICPA SOC 2 (Type 2) reports, or AICPA SOC 3 reports. Bidders must also identify all third parties with access to government information, describe audit logging practices and retention periods, detail encryption measures for data at rest and in transit, and provide an authorized representative's signature affirming the accuracy of submitted information. The questionnaire emphasizes that responses include all security-related submissions made with the bid and must remain compliant throughout the contract term.
