About this file
This Scope of Work document, prepared by SUNY Downstate Medical Center (also known as SUNY Downstate Health Sciences University) in Brooklyn, New York, outlines design and construction requirements for evacuation stations supporting Med-Sled deployment and patient evacuation procedures at University Hospital at Downstate. The project involves installing Med-Sled anchors, wall-mounted hangers, and related hardware across multiple floors (2nd through 8th) of the facility. The work requires installation of sled anchor systems at designated evacuation stations, wall brackets with specific dimensional requirements, and hangers capable of supporting both adult and bariatric patients. The project includes detailed technical specifications, floor plans identifying evacuation station locations at various staircases, installation instructions for hollow block wall anchors using 3/8" LDT anchor bolts, and configuration details for two stairway layout types. All dimensions must be verified in the field, and contractors must provide alternate anchor locations if field conditions vary from the design specifications. The project is time-sensitive, with the Department of Emergency Preparedness indicating that the work cannot be completed by internal Facilities Management and Development staff within the required timeframe.
The procurement is being conducted by SUNY Downstate with assistance from their Procurement department, with quotes to be submitted to Contract Officer Shirene Mosley. The project is funded through Emergency Preparedness Grant Funds managed by Hospital Finance and should not be charged against the Emergency Preparedness operational budget. No specific Service-Disabled Veteran-Owned Business (SDVOB), Minority/Women Business Enterprise (MWBE), or Disadvantaged Business Enterprise (DBE) contracting goals are established for this procurement (all listed at 0.00%). The selected vendor must possess the qualifications, capacity, and availability to complete the work in accordance with hospital policies and safety requirements while coordinating with Emergency Preparedness, FM&D, Hospital Finance, and other relevant departments. A preliminary price quote from FM&D has been provided as reference pricing to facilitate vendor identification and evaluation.

Secure Anchor point for Block or Grout-filled hollow wall construction (Designed specifically for Hollow Block or Grout-filled Block and may also be used with Solid Concrete)
INCLUDES:
(1) 71/2” Steel Wall Bracket
(2) 3/8” LDT Anchor Bolts
(1) Instruction Sheet
MOUNTING INSTRUCTIONS:
Figure 1
• Center of Bracket should be mounted approximately 30” or more back from the top step and 30” up from the floor.
Front edge is approximately 27” up from the floor
Figure 2
• Hold bracket flat on wall surface on a 45 degree angle and mark the 2 corner holes exactly. Mount bracket on the same wall as railing to achieve shear strength (Exhibit A)
Figure 3
• Carefully drill 5/16“ (7.9mm) diameter hole. Masonry bit is needed for Hollow, Grout-filled or Solid Concrete Block wall. (Do not use excessive pressure. Do not use Hammer Drill on Hollow Block wall. Do not let drill wander or walk from markings.)
• Place wall bracket over holes. Insert 3/8” LDT bolt and hand-tighten until bolt head becomes flush against wall bracket, then stop! (See Figure 3) Use 3/8”x 2 1/2” LDT bolt included
• Maximum empirical clamping force for the LDT bolt is only 8 foot pounds of torque. Do not over tighten
• Use both LDT bolts only, one on each end of Bracket
NOTE: LDT Anchors are “Self-Tapping” and drill holes must be 5/16 only and can only be tapped 1 single time. You cannot take bolts out and reinstall. Doing so will weaken the bolt threads
Product Warnings:
• Must be installed by qualified personnel
• All bolts must be properly positioned tight against plate and flat against wall
CAUTION:
• Have qualified personnel routinely inspect wall brackets to confirm that Wall Bracket is securely attached and maintained
• If plate is loose, retighten bolt or relocate and redrill holes

Department of Emergency Preparedness
Statement of Need
External Vendor Services for Emergency Preparedness Work Date: June 24, 2026
Prepared by: Carlos Cruz, Director, Emergency Preparedness
1. Purpose The purpose of this Statement of Need is to request procurement support to identify and engage an appropriately qualified external vendor to complete time-sensitive Emergency Preparedness work that cannot be performed by Facilities Management and Development (FM&D) within the required timeframe.
2. Background A preliminary price quote for the requested work was provided by FM&D. Although FM&D evaluated the work, it is unable to complete the project in a timely manner.
Delaying the work could adversely affect the hospital's preparedness, operational readiness, and ability to meet the intended project timeline.
Accordingly, an external vendor is required to complete the requested work. Procurement assistance is needed at this stage so that an appropriate vendor can be identified, evaluated, and engaged in accordance with applicable procurement requirements.
3. Identified Need The Department of Emergency Preparedness requires an external vendor with the qualifications, capacity, and availability to perform the requested work within the required timeframe. The selected vendor must be able to:
Review the project requirements and confirm its ability to perform the work.
Provide the necessary labor, materials, equipment, and technical expertise, as applicable.
Coordinate with Emergency Preparedness, FM&D, Hospital Finance, and other relevant hospital departments.
Complete the work in accordance with the approved scope, timeline, hospital policies, and safety requirements.
4. Procurement Rationale FM&D is unable to complete this work within the necessary timeframe. Engaging an external vendor is therefore necessary to avoid further delay and to support completion of this Emergency Preparedness project. The FM&D quote is provided as preliminary pricing information; Procurement is requested to use the requisition to begin identifying a qualified vendor capable of performing the work.
5. Funding Rationale This purchase should be charged to Emergency Preparedness Grant Funds managed by Hospital Finance. The work supports grant-funded emergency preparedness activities and is not included in the Department of Emergency Preparedness OTPS budget.
Accordingly, this expense should not be charged against the Emergency Preparedness OTPS allocation. Hospital Finance should apply the appropriate Emergency Preparedness grant funding source and account coding after confirming allowability and available funding.
6. Next Steps Upon approval of this Statement of Need and submission of the requisition, Procurement should initiate vendor outreach and solicitations as appropriate. Emergency Preparedness will support Procurement by providing the available FM&D quote, project requirements, operational considerations, and any additional documentation needed to facilitate vendor selection and timely completion of the work.
