Interior treatment includes wall void drilling, liquid chemical injection, and termiticide dust application across five designated areas: two offices, a bathroom, a lunch room, and a storage room. Exterior treatment involves continuous trenching and drilling along the entire building foundation perimeter to footer depth, supplemented by rod injection through hard surfaces at 12-inch intervals to establish a complete chemical barrier. The project is estimated to require 64 field hours across 4 days, staffed by 2 licensed technicians working during normal business hours. The total project cost is $18,726.00, with materials and chemicals comprising $8,967.00, labor costs of $3,800.00 (64 hours at $59.38 per hour), equipment and tool usage at $1,100.00, restoration materials at $680.00, site mobilization at $980.00, post-treatment cleaning at $950.00, and contingency supplies at $2,249.00. Payment terms require 50% due upon mobilization on Day 1 and the remaining 50% due upon project completion. EXECUTIVE SUMMARY

Chicago Pests LLC is pleased to present this comprehensive termite treatment proposal for the commercial facility located at 705 West Wilson Street, Rushville, IL. This proposal has been developed following a thorough on-site assessment of the structure and reflects our team's 15+ years of specialized subterranean termite management experience across commercial, institutional, and government properties throughout Illinois.

Our licensed technicians have identified active and high-risk termite pressure points throughout five interior rooms and along the full exterior foundation perimeter of the building. The treatment plan outlined in this document employs a dual-method approach — combining precision liquid termiticide injection into wall voids with exterior perimeter drilling and chemical application — providing the most complete protection available in the industry today.

Chicago Pests LLC holds all required state licenses and certifications to perform termite extermination and structural pest control services for government and commercial clients in Illinois. We are committed to delivering professional, minimally disruptive treatment that protects your facility, your staff, and the integrity of the structure.

SCOPE OF WORK & TREATMENT AREAS

The following treatment zones have been identified at 705 West Wilson Street, Rushville, IL, based on the site inspection and termite activity assessment:

Interior Treatment Areas

■ Office 1 — Wall void drilling, liquid chemical injection, and termiticide dust application throughout all interior wall voids and perimeter walls

■ Office 2 — Full wall void treatment including inspection of floor joist and sill plate areas; drilling, injection, and dusting of all accessible voids

■ Bathroom — Targeted wall void treatment with special attention to all plumbing penetration points, moisture-prone zones, and any active mud tube evidence

844-720-2847

Chicago Pests LLC | Confidential Service Proposal Page 2

■ Lunch Room — Complete perimeter wall void drilling, liquid injection, and dust application including shared walls and any areas with prior moisture exposure

■ Storage Room — Full interior wall void treatment; inspection and treatment of any cellulose material contact points, floor assemblies, and structural voids

Exterior Treatment Areas

■ Full Perimeter Trench & Drill — Continuous trenching and drill-and-treat application along the entire building foundation; trench dug to footer depth and treated with termiticide before backfilling; supplemental drilling through concrete or hard surfaces at 12-inch intervals

■ Exterior foundation soil injection using termiticide at prescribed label rates to establish a complete chemical barrier from ground level to footer depth

■ All exterior drill holes to be properly backfilled and patched following treatment to restore the surface to its pre-treatment condition

TREATMENT METHODOLOGY

Chicago Pests LLC employs only EPA-registered termiticides and follows all label requirements, Illinois Department of Public Health guidelines, and industry best practices established by the National Pest Management Association (NPMA). Our treatment protocol for this project consists of the following methods:

1. Interior Wall Void Drilling & Liquid Injection Technicians will use rotary drill equipment to create precisely placed injection ports in the base of interior walls (typically 2" above the floor line) at 12-inch intervals throughout the designated treatment zones. A liquid termiticide solution will be injected under controlled pressure into wall voids, treating the full cavity and any accessible void spaces within the wall structure. This method directly contacts termite galleries, colony members, and foraging workers within the structure.

2. Interior Wall Void Dusting In conjunction with liquid injection, a registered termiticide dust will be applied into the same wall voids via injection ports. Dust particles adhere to termite exoskeletons and are carried back to the colony, providing secondary broadcast elimination through grooming behavior — a highly effective supplement to liquid treatment that ensures maximum colony impact in areas where liquid penetration may be limited.

3. Exterior Foundation Drilling & Soil Treatment Along the full exterior perimeter of the building, technicians will trench the soil to the foundation footer depth and treat the trench with termiticide before backfilling. Where concrete, asphalt, or other hard surfaces are present, holes will be drilled at 12-inch intervals and termiticide injected directly into the soil column via rod injection. This creates a continuous treated zone in the soil — the primary pathway subterranean termites use to access the structure from below-grade colony systems. Termites attempting to cross this barrier encounter lethal doses of termiticide, which are also transferred non-repellently throughout the colony population.

Why This Dual-Method Approach is Critical Subterranean termites (Reticulitermes flavipes), the most destructive species in Illinois, construct mud tubes from the soil up through foundations, floor assemblies, and wall voids to reach cellulose food sources within the structure. A treatment strategy that addresses only one vector — either interior or exterior — leaves the colony viable and the structure at continued risk. Our combined interior injection/dusting and exterior soil barrier method ensures the colony is attacked from multiple directions simultaneously, maximizing elimination effectiveness and providing long-term residual protection.
