PROCUREMENT REQUEST INFORMATION

Contract is to be awarded and work to

SCOPE OF WORK: Furnish all necessary materials, labor, equipment, tools, and supervision to replace the existing Runway 26L glide slop shelter foundation at Gerald Ford International Airport, Grand Rapids, Michigan.

CONTRACT SCHEDULE:

begin based on contractor schedule and approval of submittals.

PERFORMANCE TIME: 30 calendar days

SHUTDOWN SCHEDULE: Place the Glide Slope on Facility Shutdown Schedule. Allow 30 days for glide slope shutdown.

The PAPI and RVR also need to be placed on the Facility Shutdown Schedule, since the power is supplied from the glide slope shelter.

CONTROL: N.A.

POWER: N.A.

LEASE OR LICENSE: N.A.

AIRPORT MANAGER'S APPROVAL: Send three sets of the project plans, specifications, and construction safety plan to the Airport Manager for his written approval.

LETTER TO AIRPORT DISTRICT OFFICE: N.A.

LETTER TO AIR TRAFFIC OFFICE: Send one set of the project plans, specifications, and construction safety plan to the

ATO.

FLIGHT CHECK: Flight check required upon projection completion.

MATERIALS: All materials, equipment, and labor required to complete the job are to be Contractor furnished.

INSPECTION: Contracting Officer’s Representative (COR) will not be on a per diem basis.

PRECONSTRUCTION: The Contractor shall attend a preconstruction conference prior to starting work at a location agreed upon between the Contractor and Contracting Officer.

JUSTIFICATION: The existing shelter foundation has heaved, causing the shelter to be out of plumb and stressing the electrical wiring and conduits connected to it.
