About this file

This is a Cost Proposal Attachment for a technical assistance contract issued by the Minnesota Department of Health to support the Rural Health Transformation Program (RHTP). The vendor will provide coordinated technical assistance to rural Minnesota healthcare organizations and Tribal Nations participating in the program, with services delivered in both virtual and in-person formats. Key deliverables include developing and implementing a one-to-one technical assistance provision plan within the first three months, establishing collaborative networking opportunities, creating written technical assistance materials, identifying existing tools and best practices, researching promising practices as approved by MDH, developing culturally relevant implementation materials, and providing bimonthly summaries to MDH highlighting technical assistance requests, applied solutions, successes, and risk areas. The vendor must participate in monthly meetings with MDH staff, engage in quarterly collaborative partnerships with other RHTP technical assistance vendors, and submit an annual progress report. Technical assistance should be available to a minimum of 125 grantees at least once monthly per organization.

The cost proposal must detail estimated hours and hourly rates for all deliverables across the initial contract period, with all costs including compensation, travel expenses, equipment, supplies, and administrative expenses incorporated into the hourly rates; no separate travel reimbursement will be provided. Responders should develop cost estimates based on their proposed approach and anticipated level of effort, accounting for potential variation in the number of organizations receiving assistance throughout the contract period. The proposal must remain open for acceptance until contract execution, solicitation cancellation, or 180 days after the due date, whichever occurs first. Cost proposals must be submitted as separate documents from technical proposals with no cost information included in the technical submission.

ATTACHMENT C: COST PROPOSAL

Responders must submit “Attachment C – Cost Proposal” form as their cost proposal.

Responders must provide cost information for all deliverables identified in the Tasks and Deliverables section below. Responders may propose additional deliverables if they believe they are necessary to successfully complete the work; however, all required deliverables must be included in the cost proposal. Detail the number of hours estimated to complete each deliverable and all hourly rates used, and calculate the total cost of each deliverable. For ongoing activities, Responders should estimate the total hours and costs anticipated for the entire initial contract period.

The hourly rates must include all costs, including but not limited to: compensation, travel expenses, equipment, supplies, and administrative costs including indirect costs and staff time related to management of the contract.

Travel expenses, including mileage, lodging, meals, and other travel-related costs, must be included in the proposed hourly rates and/or deliverable costs. No separate reimbursement for travel expenses will be provided under the resulting contract.
