The contract is estimated to run for 60 months, with an anticipated total of approximately 200 hours of service, involving medical screenings for staff at multiple locations throughout Illinois. The vendor will be required to perform comprehensive respirator medical evaluations and fit testing to meet OSHA respiratory requirements, specifically focusing on assessing and fit testing staff for N95 masks. Services must be performed at designated facilities in Springfield, Decatur, O'Fallon, and Effingham, with testing conducted during business hours (Monday through Friday, 8:30am-5pm CST).
The RFQ specifies mandatory qualifications including the vendor being an Environmental & Occupational Health Provider with an Occupational Medicine Physician and Occupational Health Nurse on staff. Estimated annual volumes include 100 new hire initial questionnaire reviews/fit tests, with potential additional services such as physical medical evaluations, EKGs, pulmonary function tests, and chest x-rays. Pricing will be submitted through BidBuy, with quotes required to remain valid for 90 days and include all expenses. The selection criteria emphasize that the vendor must meet all mandatory qualifications and offer the lowest price, with IDPH reserving the right to reject all offers. Subcontracting is not permitted, and vendors must commit to emailing reports to IDPH on a weekly basis.
