PERFORMANCE WORK STATEMENT
MEDICAL WASTE REMOVAL 30th MEDICAL GROUP VANDENBERG AFB, CA
12 Sept 2016
SCOPE
The Contractor shall provide all personnel, equipment, tools, materials/supplies, transportation, supervision, other items and services necessary to perform all operations for the collection, transportation, and disposal of medical waste from the 30th Medical Group (MDG), Vandenberg Air Force Base, CA.
CONTRACTOR PERFORMANCE
1. Transportation and disposal of medical waste shall be in accordance with all Federal, State, Local, and Air Force regulations. Collection and transportation of wastes shall be made with minimal divergence from the schedule established with minimum disturbance and maximum protection of personnel and government property.
2. Contractor must provide license numbers and copies of permits as indicated to which this service pertains, to include the following:
a. Medical Waste Permit issued by the state of California (need copy of permit).
b. Hazardous Waste Transporter Registration (need copy of permit).
c. Medical Waste Transfer Station and Treatment Facility number.
d. Waste water discharge license number.
CONTRACTOR FURNISHED EQUIPMENT / SUPPLIES
1. The contractor shall provide all necessary containers and labels required for the accumulation of medical waste products. The waste materials will be packaged in Contractor supplied containers by government personnel and will be picked up at a single location across from building 13850 at the medical waste storage area. Wastes to be disposed of include laboratory wastes, infectious wastes (anything that comes in contact with blood or body fluids), sharps containers, surgical specimens, isolation wastes and pathological /Chemo wastes.
2. The contractor will furnish all necessary collection containers in a manner that will ensure that there will always be six (6) each 90 gallon containers with wheels, on hand at the medical facility at all times. Containers must be labeled with biohazard labels on all sides, so as to be seen from any angle.
3. Vehicles shall be operated in accordance with base rules and regulations while in the base area.
4. All of the contractor's equipment shall be equipped with proper safety devices and shall be in safe operating condition. The contractor shall perform all work in a fire safe manner in accordance with fire prevention standards of Vandenberg Air Force Base. The contractor is responsible for compliance with all Federal and State safety directives and is subject to OSHA inspections while on base.
5. Permission for contractor personnel and vehicles to enter Vandenberg Air Force Base is subject to approval by the Contracting Squadron and the Security Forces Squadron. The contractor shall be responsible for obtaining all necessary cards, passes, buttons, decals or other items required to apply for vehicle and individual identification media at the Security Forces Squadron, Pass and ID Section. Upon completion or termination of the contract or an individual's employment on the contract, the contractor shall retrieve all identification and access media issued and surrender it to the Security Forces, Pass and ID Section.
SCHEDULE OF OPERATIONS
1. The point of collection of medical waste by the contractor shall be across from building 13850 at the medical waste storage area, 30th MDG, Vandenberg Air Force Base.
2. Pick-up shall be every Tuesday during normal business hours (7:30 a.m. to 4:30 p.m.). Any changes to this schedule must be approved by the Contracting Officer and by the 30th MDG Facilities Management Team prior to implementation. The contractor shall pick-up all containers used since last pick-up.
3. In accordance with the Medical Waste Management Act, Section 118280 (d)(1)(A); If the person generates 20 or more pounds of biohazardous waste per month, the person shall not contain or store biohazardous or sharps waste above 0 degrees Centigrade (32 degrees Fahrenheit) at any onsite location for more than seven
(7) days without obtaining prior written approval of the enforcement agency.
4. The medical waste stored at the 30th MDG is normally stored at above 0 degrees Centigrade (32 degrees Fahrenheit), so that containers ready for disposal shall not be held by the 30th MDG for more than seven (7) days without the written approval of the enforcement agency. Pick up will occur on Tuesdays during normal business hours (7:30 a.m. to 4:30 p.m.). This schedule will not be deviated from even in the event of a holiday or base down day. Therefore, the time between pick-ups, shall not exceed seven (7) days. If for any reason the contractor cannot complete the medical waste pick-up due to unforeseen equipment failure, etc., the contractor will immediately contact the 30th MDG Facilities Management Team. During weeks where a holiday and or down day occur, a contractor may pick up the medical waste more often than once per week if they obtain prior approval and coordination from the 30th MDG Facilities Management Team.
DOCUMENTATION
1. The Contractor shall provide a copy of the manifest/certificate of destruction or other equivalent Department of Transportation (DOT) shipping papers to the COR upon shipment of medical waste containers which include the following information:
a. Regulated Medical Waste Transporter number
b. Regulated Medical Waste Handling Permit number
c. U.S. DOT number
d. Transporter’s company name, mailing address, telephone number
e. Quantity of medical waste being shipped
f. Date of shipment.
2. The Contractor shall provide the completed manifest/certificate of destruction to the COR within 30 days of the medical waste reaching its final destination.
3. The deliverables from this contract will be used to support regulatory recordkeeping requirements for regulated medical waste disposal at Vandenberg Air Force Base.
