The form requires offerors to declare whether they qualify as a minority business enterprise, defined as a business at least 50 percent owned by minority group members or, in the case of publicly owned businesses, at least 51 percent owned by minority group members. Offerors must indicate their MBE status by checking the appropriate box corresponding to the ownership classification of African American, Asian American, Hispanic, Native American, Female, Disabled, or None.
This attestation form serves as a mechanism to track and potentially advance diversity and inclusion goals within the procurement process. The form does not specify pricing, contract terms, performance requirements, or other substantive procurement details; rather, it functions solely as a certification document to document the demographic composition of the business ownership for the transportation routing system RFP.
