About this file
This is a Service Level Agreements Schedule D document for the Michigan Department of Corrections (MDOC) Release Day Transportation RFP contract, outlining performance metrics and accountability measures for contracted staff and subcontractors. The agreement establishes five key performance metrics: mandatory MDOC training completion by September 30th annually with 100% compliance required; responsiveness to Corrective Action Plan requests within specified timeframes at 100% compliance; Law Enforcement Information Network (LEIN) clearance for staff entering correctional facilities or accessing employee records prior to contract start and annually thereafter; timely and accurate invoice submission within 15 calendar days of each monthly invoicing period with 100% accuracy required; and proper adherence to referral decline procedures and timely completion of coordinated transportation requests at 100% compliance. Data verification occurs through MDOC training databases, corrective action documentation, LEIN forms and databases, monthly invoices cross-referenced with general ledgers and offender files, and correspondence with MDOC staff.
Service level credits are assessed as financial penalties applied against contractor payments or payable directly to the State of Michigan within 10 calendar days upon demand. Specific credit amounts include $250 per non-compliant staff member plus an additional $500 if over 30% fail training deadlines; $500 per business day for delayed corrective action plan submissions; $250 per LEIN clearance violation; $500 per month of untimely or inaccurate invoicing; and credits equal to the cost of each coordinated transportation referral for which the contractor fails to follow denial procedures, provides late pickup, or no-shows. The MDOC Contract Manager has discretion to review extenuating circumstances before assessing credits. All metrics are subject to annual audits or monthly reviews conducted by MDOC to ensure ongoing compliance throughout the contract term.
