About this file
This is a Solutions Summary template required for proposals submitted to the ARPA-H Restorative Engineered Sleep Treatment (REST) Innovative Solutions Opening (ISO).
Proposers must submit a single solution summary covering Technical Area 1 (TA1) and/or Technical Area 2 (TA2), with maximum page lengths of six pages for a single TA or eight pages for both TAs. The template mandates 12-point Avenir Next LT Pro font, with 8 or 10-point font permissible for figures, tables, and charts. All submissions must be in English. The page limit applies only to Sections 1-4 and excludes the cover page, Basis of Estimate, Cost Share Strategy, and Citations sections.
The template requires completion of a comprehensive cover page containing prime organization details, SAM.gov UEI, key personnel and points of contact information, technical areas addressed, animal and human subject involvement declarations, total ARPA-H funding requested, and place(s) of performance. The submission must include sections on Concept Summary, Innovation and Impact (with quantitative metrics comparing proposed technology to current and emerging technologies), Proposed Work (deliverables, milestones, technical approach, supporting scientific rationale, and identification of technical risks), and Team Organization and Capabilities (including roles, responsibilities, and key personnel with anticipated level of effort). A detailed Basis of Estimate encompassing all costs and resources is required, including direct labor, materials, supplies, equipment, travel, indirect costs, and profit/fee. TA1 proposers must submit a preliminary cost share strategy describing anticipated sources and sustainability approach. Sub-awardees and consultants must be identified with their organization type and technical point of contact. All references cited must include complete bibliographic information with persistent identifiers such as DOI or PubMed ID.

ATTACHMENT A: SOLUTIONS SUMMARY MODEL
Use of this template is required for Solution Summaries submitted to the REST ISO
All submissions must be written in English with 12-point Avenir Next LT Pro font. Font sizes of 8 or 10 point may be used for gures, tables, and charts. Delete all formatting and content instructions prior to submission. Instructions and content recommendations are displayed in blue font and should be deleted prior to Solution Summary submission.
Proposers are required to submit a Solution Summary in advance of a full proposal. Proposers will submit a single solution summary covering TA1 and/or TA2. The total length of the solution summary should not exceed 6 pages if submitting for a single TA, 8 pages if submitting for both TA1 and TA2.
NOTE: Each template attempts to provide proposers with guidance on development of submission content. It is the proposer’s responsibility to ensure proposal content meets the evaluation criteria stated in the ISO. Content recommendations are displayed in blue font and should be deleted prior to proposal submission.

1. CONCEPT SUMMARY
Describe the Solution Summary concept with minimal jargon and explain how it addresses the REST Technical Area(s).
2. INNOVATION AND IMPACT
Clearly identify the outcome(s) sought and/or the problem(s) to be solved with the proposed technology concept. Describe how the proposed effort represents an innovative and potentially revolutionary solution to the REST Technical Area(s). Explain the concept’s potential to be disruptive compared to existing and emerging technologies. To the extent possible, provide quantitative metrics in a table that compares the proposed technology concept to current and emerging technologies which may include:
• A progression of increasingly complex technical challenges.
• State of the art / emerging technology “baseline.”
• Aggressive metrics for each year of the proposed project.
• Summary of specic outcomes from the proposed project or concept.
3. PROPOSED WORK
Describe the nal deliverable(s) for the project, key interim milestones, and the overall technical approach used to achieve project objectives. Provide a concrete research plan at a level of detail that can be used to assess for novelty, scientic and technical merit, and potential feasibility. Discuss alternative approaches considered, if any, and why the proposed approach is most appropriate for the project objectives. Describe the background, theory, simulation, modeling, experimental data, or other sound engineering and scientic practices or principles that support the proposed approach.
Provide specic examples of supporting data and/or appropriate citations to the scientic and technical literature. Identify adoption challenges to be overcome for the proposed technology to be successful. Describe why the proposed effort is a signicant technical challenge and the key technical risks. ARPA-H expects projects to have substantial technical risk, counterbalanced with the potential for transformative impact, which must be clearly addressed within the submission.
If applicable, describe how the proposed work will interface with the broader REST ecosystem, including the Foundational Sleep Health Modeling and Discovery effort and the Research Integrator and IV&V effort.
4. TEAM ORGANIZATION AND CAPABILITIES:
4.1 Indicate the roles and responsibilities of the organizations and key personnel comprising the Project Team.
4.2 Provide the name, position, and institution of each key team member and describe in 1-2 sentences the skills and experience they bring to the team. Key team members include the individuals who are expected to contribute to the scientic development or execution of the project in a substantive, measurable way. Include anticipated level of effort for all key personnel.
5. BASIS OF ESTIMATE (BOE):
5.1 Identify a BOE that encompasses all costs and resources for the entire proposed project team. The BOE should include direct labor, materials, supplies, equipment, travel, prot/fee, indirect costs, and any other relevant costs associated with the proposed project.
5.2 The BOE should identify whether resource sharing applies. Please further describe your value proposition and/or return on investment for this effort for the U.S. taxpayer beyond scientic or technical outcomes. The following table may be used:
Categories Amount Direct Labor (fully burdened) Labor hours Subcontracts/Consultants Materials Equipment Travel Other Direct Costs Indirect Costs Prot Total (including all phases) Cost Sharing (if applicable)
6. PRELIMINARY COST SHARE STRATEGY (TA1 PROPOSERS ONLY)
For Solution Summaries addressing TA1, describe the preliminary cost share strategy, including anticipated source(s) of cost share, when such support is expected to become available, and how the proposed cost share approach supports transition, commercialization, and long-term sustainability.
7. REFERENCES CITED – NOT INCLUDED IN THE PAGE LIMIT
For any references cited in the Solution Summary, provide uniquely identifying bibliographic information in this section, including rst author, title, source (e.g., journal or book), year of publication, volume and page numbers (if applicable), and a persistent identier such as a DOI or PubMed ID, where available.
