About this file
This is a Quote Submittal Checklist from the U.S. Embassy detailing required documents and information that must be included with vendor quotes submitted to Procurement_Box4@state.gov for a maintenance contract for two CATISO SCADA systems.
The checklist requires: completed SF-1449 form with specified blocks filled in; completed pricing tables with currency specified; Project Manager details with English language capability; proof of established business operations; past performance references for the last 3 years (UK or international experience); evidence of necessary resources and capabilities; required licenses and permits; a strategic work plan including equipment/supplies details and quality assurance measures; proof of required insurance; and SAM registration for quotes $30,000 or higher. The document emphasizes that all requirements must be met for each new solicitation regardless of previous work with the U.S. government, and incomplete submissions will be deemed technically unacceptable.
