Quality Assurance Surveillance Plan

For Repair of the F-16 APG-68 Traveling Wave Tube

Purpose: The purpose of this Quality Assurance Surveillance Plan (QASP) is to identify the methods and procedures the Government will use to ensure it receives the services to be put under contract as identified in the Performance-based Work Statement (PWS) for the F-16 APG-68 Traveling Wave Tube. This plan is established to evaluate contract requirements and contractor's actions during the life of the contract and is designed to provide an effective method of measuring the quality and improvement of the component repair effort.

All specific contractor requirements shall be identified / authorized by individual delivery orders and the PWS. The Defense Contract Management Agency (DCMA) is delegated quality assurance responsibilities under this QASP. DCMA will act as the Contracting Officer

Representative (COR) and participate in the surveillance of the contractor IAW their approved surveillance plans.

Award of requirement will result in a 2 year, firm fixed price, contract for the repair of the F-16 APG-68 Traveling Wave Tube. Estimate of requirements is 20 assets per year for a total of 40 assets over the life of the contract.

Roles and Responsibilities of the Government Multi-function Team: The Multi-functional

Team (MFT) has developed this QASP as an avenue to develop a solid communications tool in which the government and contractor will collectively ensure that quality products are returned to the Air Force inventory within cost and schedule. The multi-functional team will remain a collective group throughout the life of the contract. Team members are representative of functional areas. The MTF members have specific functional and/or technical responsibilities that may include but are not limited to:

Contracting Officer (CO) -Contract management

Program Manager (PM) - Requirements, Asset delivery schedule, CDRLs, Quality Assurance

Production Management Specialist (PMS) -Asset delivery schedule, CAV reporting

Engineer - Technical data

Equipment Specialist (ES) - Tech/Maintenance Data, Defense Repair Information Logistics

System (DRILS)

Material Manager (MM) - Requirements, Asset delivery schedule, Inventory posture

DCMA Quality Assurance Specialist (COR) - Quality assurance

Performance Assessment of the Contractor:

Surveillance of contractor actions will be inspected and monitored IAW the DCMA's

Surveillance Plan.

A summary of any unacceptable ratings will be recorded and provided to the CO at Ogden OO-

ALC.

The contractor is responsible to ensure all-inclusive management of the quality control requirements are accomplished as outlined in the contract. This includes the establishment of a viable quality control program, detailed inspection programs, monitoring actions and effective corrective actions.

The contractor will be evaluated by surveillance of members of the MFT through inspections and/or reviews of required documents.

Surveillance will be accomplished by DCMA's surveillance plans. DCMA will review and document all complaints along with resolution of such complaints with the documentation becoming a permanent part of the surveillance records.

If at any time during the surveillance period, the results of the surveillance show that the number of unacceptable observations during the period exceeds the performance threshold, and it is determined that it is not government caused, the DCMA ACO notifies the CO in writing. The

CO and Contractor will evaluate the situation and negotiate consideration for unacceptable performance. The authority used is the Inspection of Services Clause.

All negative comments/actions/reviews will be dealt with IAW contractual arrangements.

Performance Management of Contract:

If at any time during the period of performance, the results of surveillance show that the number of unacceptable observations during the period exceeds the performance threshold, and it is determined that it is not government caused, the DCMA ACO will notify the assigned CO. The

CO will evaluate the situation discuss and discuss remedies to improve performance, negotiate consideration for unacceptable performance, and otherwise assert the Government's contractual rights when necessary.
