About this file
This file contains Questions and Answers (Q&A) from a Request for Information (RFI) issued by the Massachusetts Department of Mental Health regarding competency restoration services training. The RFI, identified as BD-27-1022-DMH08-8210C-130916, seeks pricing and program information from qualified vendors to deliver a training series on evidence-based manualized competency restoration services. The initiative is designated as a priority for the state fiscal year beginning July 1, 2026, with written questions from interested vendors submitted through July 10, 2026, and DMH responses posted by July 17, 2026. The RFI specifically targets training delivery and implementation support for DMH staff to develop internal capacity to deliver competency restoration services directly to individuals for whom the court has determined that mental illness or mental defect may affect their competence to stand trial or criminal responsibility. DMH expressed a preference for in-person, real-time training conducted at DMH sites, though specific training dates have not yet been selected. Training will focus on informing DMH about appropriate staff disciplines, prerequisite skills, and best practices in competency restoration, with the subsequent RFP to follow based on RFI responses.
DMH seeks cost estimates for material development, educator staff time, and other relevant service categories from vendor responses. This is a new initiative with no incumbent vendor, and all materials developed at DMH's expense will be owned by the department. The current population committed for incompetence to stand trial consists of approximately 145 individuals, with this number subject to constant flux from court admissions and discharges. DMH is not currently collecting baseline metrics for individuals restored to competence but intends to conduct this analysis and is interested in learning about national measures and trends for evaluating intervention impact. The department may subsequently consider implementing a "train the trainer" model to develop internal DMH staff as trainers. This RFI is limited to training, implementation support, and evaluation of competency restoration service delivery; competency evaluations themselves and external staff to deliver direct interventions are explicitly outside the scope of this initiative.

Answer This is a new initiative. The purpose of this RFI is to gather information to assist us in understanding associated costs.
This initiative is a priority for the current state fiscal year (7/1/26 to 6/30/27).
This is a new initiative. There is no current vendor.
Yes. DMH is seeking a vendor to provide staff training and implementation support for DMH staff to deliver competency restoration services directly.
DMH has not identified any barriers to the implementation of a standardized approach to competency restoration within existing statutory authority and there are no anticipated legisltive or policy changes needed at this time.
The purpose of this RFI is to obtain "pricing and program information from qualified vendors to deliver a training series on an evidence-based manualized competency restoration service". The dates of the training series have not yet been selected. The intended recipient population for the competency restoration services are Individuals for whom "the court has reason to believe that such observation and further examination are necessary in order to determine whether mental illness or mental defect have so affected a person that s/he is not competent to stand trial or not criminally responsible for the crime or crimes with which s/he has been charged.”
The purpose of the RFI is to obtain information from subject matter experts on "what disciplines of staff have you trained to provide competency restoration services … [and] Are there prerequisite skills, background, or education for staff to attend?"
The focus of this RFI is training, implementation support and evaluation of the delivery of competency restoration services. Competency evaluations are not within the scope of this RFI.
Generally DMH prefers in-person, in real time, training over an asynchronous virtual training model.
Responses to this RFI will inform DMH's decisions about the number of trainees and the timeline.
DMH expects that training sessions would be conducted at DMH sites.
With this RFI, DMH is seeking information on estimates of the cost of material development, educator staff time, and any other relevant price categories for your service.
See response to Question #1
See response to Question #3
Upon reviewing responses to this RFI, DMH will determine whether or not to proceed with a subsequent request for response to deliver a service.
All materials developed at the expense of DMH are owned by DMH.
At this time, the purpose of this RFI is to clarify elements and costs of developing and delivering training in competency restoration for DMH staff. DMH may subsequently consider a 'train the trainer' model for DMH staff to become trainers.
This is a training initiative. This is not an initiative to hire external personnel to deliver an intervention.
See response to Questions #7 and #13.
The purpose of this RFI is to obtain information about current best practices in competency restoration. In your response, please indicate what, if any, psychological tests and their administration constitute a core compenent of the competency restoration intervention you would recommend.
See response to Questions #13.
See answer to Question #13
The scope of the current RFI is for DMH operated inpatient services.
At this time we do not collect a clear baseline metric for individuals returned to competence, however this is an analysis that we intend to conduct. With this RFI, DMH is interested in what measures are currently in use nationally to evaluate the impact of the recommended intervention and current national trends with attention to characteristics that predict treatment response.
We are still in the process of analyzing current statewide length of stay for individuals on a commitment status of incompetent to stand trial.
At the time of this response, the current population committed for incompetence to stand trial is roughly 145 individuals. This number is constantly in flux with discharges and new admissions from Courts and or Bridgewater State Hospital, however the ICST population remains a significant portion of all committed patients. There is no backlog for ICST individuals.
