About this file
This is a Prevailing Wage Weekly Statement of Compliance form issued by the Massachusetts Department of Labor Standards (DLS) for public works projects in accordance with Massachusetts General Law c. 149, 27B. The document establishes mandatory requirements for all contractors and subcontractors working on public works projects to maintain accurate payroll records and demonstrate wage compliance on a weekly basis. Contractors and subcontractors must submit certified weekly payroll records to the awarding authority within 15 days after completion of their portion of work, accompanied by a signed Statement of Compliance form certifying that all mechanics, apprentices, teamsters, chauffeurs, and laborers have been paid in accordance with prevailing wage rates determined under Massachusetts General Law sections 26 and 27 of chapter 149. These records must be preserved by both the contractor and the awarding authority for a period of three years from the date of project completion.
The payroll forms and Statement of Compliance documentation are available from the DLS at mass.gov/dols/pw and must be submitted to the awarding authority either by regular mail or email. The form requires certification by an authorized officer of the contractor or subcontractor who supervised wage payments, including the signatory's name, title, the employing entity, and the building or project name. This prevailing wage compliance requirement applies to the referenced project "TRD01 Replace Walk-In Cooler Refrigeration System" in Massachusetts and ensures adherence to state prevailing wage standards for all workers employed on the public works project.
