About this file

This is an Operational Response Template for the Promoting Appropriate Transitions to Home (PATHways) program, a Massachusetts state initiative administered by the Health Policy Commission (HPC) in partnership with Aging Services Access Points (ASAPs) and qualifying acute care hospitals. The template guides hospital applicants through a structured proposal process designed to establish integrated discharge planning models that facilitate patient transitions from hospital settings to home-based care. Applicants must demonstrate their hospital's capacity to embed ASAP staff members within their discharge planning processes, describe their proposed staffing models, outline staff training plans, and detail communication protocols between hospital, ASAP, and home and community-based services (HCBS) providers. The response template requires hospitals to identify their target patient populations, explain how the program will enhance service efficiency and effectiveness, and provide geographic and population needs analysis for their service areas.

The evaluation framework assigns point values across five categories: Proposed Program (40 points), Applicant qualifications and need (20 points), Measurement and Data Reporting (5 points), Budget (15 points), and Sustainability and Scalability (20 points). Applicants must demonstrate organizational financial need, describe existing relationships with ASAP partners, provide institutional leadership support documentation, and detail workflows for tracking eligible individuals, service offerings, and enrollment data. The response requires budget context and explanation of in-kind funding contributions and timing. Applicants must articulate sustainability strategies beyond the Period of Performance, including return-on-investment analysis and internal organizational buy-in approaches. The template does not specify response dates, contract terms, funding amounts, or award timelines within its content.
