About this file
This is a budget template and guidance document issued by the Massachusetts Health Policy Commission (HPC) for the Promoting Appropriate Transitions to Home (PATHways) investment program, which supports partnerships between Aging Services Access Points (ASAPs) and qualifying acute care hospitals to facilitate patient discharge to home settings. Applicants must submit budgets using the provided template that account for all expenditures during both the preparation and implementation periods. The template requires applicants to identify key financial personnel, including a Financial Designee responsible for tracking all funds and a Financial Delegate if applicable, with both individuals required to possess relevant accounting skills and maintain regular communication with the HPC throughout the period of performance.
The award is capped at $210,000, with a maximum of 70% of total program costs eligible for HPC reimbursement and a minimum 30% in-kind contribution requirement from the awardee. Reimbursable expenses include employed personnel salaries and wages (capped at $228,000 annually per FTE), fringe benefits up to 25%, subcontractor services with fringe benefits up to 25%, technology, trainings, and other approved costs, while indirect costs and travel expenses are not eligible for HPC reimbursement. The budget template requires line-item detail for personnel costs, fringe benefits, and support costs across designated categories including subcontractors, general supplies, technology, staff training, travel (in-kind only), and other expenses, with all calculations auto-populating through embedded formulas.

Budget Guidance
Budget Response Instructions
NOTE: Please review these instructions carefully. The requirements for this Budget Response may differ from those of other HPC investment programs with which Applicants may be familiar.
OVERVIEW
Applicants must submit a budget using this template that reflects all proposed expenditures to plan and implement the proposed Program. Applicants should carefully consider all costs that may be incurred in both the Preparation and Implementation Periods and propose a budget accordingly. Subject to the $210,000 award cap and the HPC’s Terms and Conditions, no more than 70% of the total Program Cost will be eligible for reimbursement by the HPC quarterly. At least 30% of the total Program Cost will be in-kind contributions from the Awardee. The HPC will determine which costs will be reimbursed by the HPC.
As indicated in Section IV of the RFP, payment of Awards will be made on a cost reimbursement basis for the Period of Performance. Subject to the HPC’s Terms and Conditions, the HPC will reimburse Awardees for expenses they incur for employed personnel salaries/wages; technology; subcontractors; and other approved costs. Please note: indirect costs are not eligible for reimbursement by the HPC and cannot be included in the proposed budget as in-kind contributions.
HOW TO COMPLETE THE BUDGET RESPONSE TEMPLATE
Please complete the “Budget” tab in this workbook. Enter text in the yellow cells only. Make sure all HPC funded line items and in-kind funded line items are in the appropriate columns. All other cells are locked, and formulas will auto-populate. Applicants that require additional rows to reflect additional line items or categories of costs should contact the HPC at HPC-PATHways@mass.gov.
