Dear Respondent, The United States General Services Administration would like to thank you for taking the time to assist us in the selection of a General Contractor for our Project. Please complete the questionnaire to the best of your ability. We ask that you keep your responses to this questionnaire confidential. The information you provide on this questionnaire will not be shared with the Offeror.

When completed, please send a scanned or electronic copy of the questionnaire to the following e-mail address:

1. Did the contractor complete the project within the original contract budget? If not, what was the reason for the difference in actual costs for the project?

2. Did the contractor cause any delays to the original project schedule? If so, what was the reason for delays?

3. Did the contractor staff the job adequately? Were key personnel adequately qualified for their positions?

4. How responsive and responsible was the contractor in administrative areas? (e.g.

submitting reports, documenting progress/problems, filing correspondence, tracking changes, responding to Requests for Information (RFI’s), processing payments, etc.)

SAFETY

5. Did the contractor address safety issues? (Developed a good safety plan, followed it, enforced it with subcontractors, posted OSHA information, etc.)

PROJECT MANAGEMENT

6. Did the prime contractor manage the subcontractors effectively? (Coordinate work, update them on changes, pay promptly, address work if performance was poor, etc.)

How well did the contractor demonstrate sensitivity to client/tenant concerns

(coordination issues, debris, noise, fumes, inconveniences, disruptions within the occupied spaces, etc.)?
