SECTION L
INSTRUCTIONS, CONDITIONS AND NOTICE TO BIDDERS

National Park Service Solicitation Number P16PS00226
1. Relevant Deadlines
A. 12:00 p.m. Eastern Standard Time on August 30, 2016 – Deadline for National Park Service (NPS) Receipt of Proposal, Binder 2, Relevant Past Performance.
B. 12:00 p.m. Eastern Standard Time on September 7, 2016 – Deadline for NPS Receipt of
Proposal Binders 1 and 3, Technical Submission Volume and Business Management Cost Proposal Volume.
2. Instructions for Proposal Submission
Proposals shall be submitted as one original plus two paper copies.
A. Hardcopy submissions
Hardcopy submissions shall be on 8 ½ x 11 paper, single-spaced. The original document shall be printed on one side of the page, copies are permitted to be printed on both sides.
Margins shall be no less than one inch on all sides. The typeface for all documents (including charts and graphics) shall be black, 11 point or larger. Graphic illustrations may be reduced for presentation purposes but must be presented on 8 ½ x 11-inch paper.
Proposal submission initially includes both written content and price. Include the solicitation number on all documents.
B. Proposal Format
Proposals shall be submitted as three separate 3-ring binders for the written proposal:
Binder 1 Technical Submission Volume Binder 2 Relevant Past Performance Volume Binder 3 Business Management Cost Proposal Volume
Each Binder shall contain a cover sheet which clearly identifies the following information:

C. Proposal Receipt
Proposals Binders 1 and 3 must be received at the following address no later than 12:00 p.m. Eastern Standard Time, September 7, 2016:

Binder 2 – Past Performance and completed Past Performance Questionnaires must be received at the above address no later than 12:00 p.m. Eastern Standard Time on August 30, 2016. Completed Past Performance Questionnaires must be submitted directly from the Offeror’s client or reference; completed questionnaires, other than Section 1 submitted by the Offeror shall be discarded without consideration.
All late proposals submissions will be handled in accordance with FAR 15.208.
All proposals must be closed and sealed when they arrive at the specified address, and must be clearly identified as SOURCE SELECTION SENSITIVE MATERIAL on the exterior packaging.
2. Proposal Content
A. Technical Submission
(1) TECHNICAL APPROACH AND SCHEMATIC PLAN
(a) The Offeror shall provide a Technical Approach that identifies:
- Preferred plan for visitor safety;
-Method for removing debris and existing roofing material from the roof to the ground;
-List nail manufacturer and stock item number for applying cedar shakes;
and
-Fall protection plan for workers.
(b) The Offeror shall also provide a schematic site plan sketch that identifies and includes the following:
-Existing building(s), sidewalks, streets, porches and stairs;
-Dimensions for any scaffolding, jersey barriers, or related equipment that extends into the parking lot;
-Location of scaffolding and work access stairs and ladders;
-Detail of scaffolding tie-in bracing if required;

-Identification of any windows that will be used for tie-in; and -Details of jersey barriers (if any) showing style, dimensions and locations.
(2) QUALIFICATIONS AND PAST EXPERIENCE
(a) The offeror shall submit a copy of the Cedar Shake and Shingle Bureau (CSSB) New Roof Construction Manual March 2015.
(b) The Offeror shall submit photographs of two previous roof projects that were completed on historic structures preferably with wood shingles.
(3) INSTALLER’S AND MANUFACTURER’S WARRANTY
The Offeror shall submit a sample of the installer’s and manufacturer’s warranty.
(4) PAST PERFORMANCE
(a) Definitions
The following definitions apply in all instances when used in this solicitation relative to Past Performance:
“Recent” Past Performance is defined as past commercial or government contract work performed as the prime contractor such that an portion of the work was executed within the 36-months immediately preceding release of the solicitation. This is a PASS/FAIL assessment based solely on the timeframe during which the Past Performance event being considered occurred. Past Performance events which fail this assessment will be discarded without consideration and will not be factored into the overall Performance Confidence Rating.
“Relevant” Past Performance is defined as the degree to which an Offeror’s past effort as a prime contract is similar in scope, magnitude and complexity to the requirements set forth in the solicitation.
“Adverse Past Performance” is defined as any information received from a third-party pertaining to an Offeror’s performance quality on a past performance event which has been determined to be both “Recent” and “Relevant” as defined above, and which results in a past performance event receiving a MARGINAL or UNSATISFACTORY.
The existence of validated Adverse Past Performance information may result in an overall Performance Confidence Assessment rating of NO CONFIDENCE for the Offerors.

(b) Past Performance Questionnaires
Past Performance Questionnaire is available in Attachment 12 to this Solicitation.
Information derived from completed and returned Past Performance Questionnaires will be used to access the Performance Quality of the identified past performance event.
Offerors shall forward Past Performance Questionnaires, in Attachment 12 along with response instructions and client authorization letter, to clients for previous roof projects that were completed on historic structures preferably wood shingles or performed with duties similar in scope, magnitude and complexity to those required under this solicitation. We understand clients may be reluctant to complete, so please prepare a cover letter to your clients explaining the importance and need to complete and return the Questionnaires, provide instructions for where information should be forwarded (see National Park Service cover letter, Attachment 12) and identify the date for when the information is due to the Government.
Since “return rates” on Past Performance Questionnaires vary widely, Offerors are encouraged to forward as many questionnaires to their clients as practicable to ensure adequate response.
The Government has no obligation to contact references who fail to return the questionnaires.
Offeror shall provide, in this Binder, a detailed listing of all personnel to whom the Offeror has sent Past Performance Questionnaires. At a minimum, this listing shall contain the following information for each individual to whom a Past Performance Questionnaire was sent:

(3) Past Performance Questionnaire
Offerors shall have their clients submit completed Past Performance Questionnaires directly to the National Park Service. The Offeror shall submit, in Binder 2, a minimum of 3 and a maximum of 5, Past Performance Questionnaires available at Attachment 12. The Offeror shall complete only the Past Performance Questionnaire Section 1 so the Government can identify which clients should return completed Past Performance Questionnaires to the Government. Completed Past Performance Questionnaires must be submitted directly from the Offeror’s client or reference to the Government address identified on Page L-2.
Information derived from the Past Performance Information Questionnaires will be used to determine the recency and relevance of the identified efforts.
In each Past Performance Information Questionnaire, it is incumbent upon the Offeror to identify the specific aspects of each past project which are relevant to the requirements of this solicitation.
For each Past Performance Questionnaire complete Section 1. The Offeror shall also submit a client authorization letter, clearly authorizing the commercial customer to release requested performance information to the Government.
(4) No Past Performance Data Available
In the event an Offeror is determined to have no past performance information which is both recent and relevant as defined above, the Offeror shall be subject to the provisions of FAR 15.305(2)(iv).
(5) Sources on Past Performance Data
Offerors are cautioned that the Government may obtain and evaluate Past
Performance information obtained from other sources beyond those identified in the Offeror’s proposal. Other sources of Past Performance information include, but are not limited to, other government contracting activities, GAO/Defense Procurement Fraud Information and Industry Publications.
C. Business Management Cost or Price Proposal
(1) Originally complete and signed Standard Form 1447. Solicitation Offer and
Award (Construction, Alteration and Repair) form shall be submitted. Complete Blocks 14, 15 and 30a through 30c.
(2) Submit Section B, Supplies or Services and Prices. Provide a detailed cost proposal which fully supports the prices set forth in Section B that will adequately establish the reasonableness of the proposal amount.

(3) All Contractors doing business with the Government must register in the System for Award Management system. If you are already registered, confirm your registration with your response, and provide you Dunn and Bradstreet Number. If you are not registered, register at https://www.sam.gov/portal/public/SAM/##11 and confirm your registration and provide your Dunn and Bradstreet Number.
(4) In those cases where an Offeror does not have full in-house capability and anticipates significant use of subcontractors to provide the needed support, the Offeror shall provide the same detail concerning the subcontractor as provided by the prime contractor. The terms of each subcontract should be fully explained.
Subcontracts must include certain general provisions and contract articles from the prime contract. Any difference in the rates proposed by the subcontractor to the offeror and the rates proposed by the offeror to the Government for the subcontract work should be fully explained.
Any proposed subcontractors required by the Offeror in connection with the services covered by the contract (excluding purchases of materials or commercial stock items) will be limited to individuals or firms that are specifically identified and agreed to during negotiations. Therefore, Letters of Commitment for subcontracts must be in place at the time of proposal submission. Provide written confirmation that Letters of Commitment are in place.
(5) REPRESENTATIONS AND CERTICATIONS
Complete Representations and Certifications identified in Section K or complete the Representations and Certifications in the System for Award Management
(SAM).
