Attachment 12 - Past Performance Questionnaire National Park Service Solicitation Number P16PS000226
National Park Service Solicitation Number P16PS00226
United States Department of the Interior National Park Service 4801A Urbana Pike
Frederick, MD 21704
IN REPLY REFER TO:
The National Service is currently in the process of selecting a Contractor to provide all labor, materials, equipment and travel necessary for completion of the USS Maine Memorial Restoration in Arlington, Virginia.
As required by the Federal Acquisition Regulation (FAR) at Paragraph 12.606, Offeror’s past performance history will play an important role in determining which Contractor will receive the contract.
As someone with first-hand knowledge of the past performance history pertaining to one of the competitors for this project, your participation is essential. The National Park Service depends on your critical evaluation of the prospective Contractor’s performance in order to ensure we are obtaining the best value for your tax dollars.
The acquisition schedule is extremely tight; therefore we need your written response no later than August 30, 2016. To assist you in preparing your response and expediting your reply, we have formulated the attached questionnaire highlighting the areas of primary interest to the National Park Service.
The questionnaire may be filled out by hand and faxed to the attention of Sheila Rushlow at (301) 663- 8032, or scanned and E-Mailed to sheila_rushlow@nps.gov.
If you have any questions concerning completion of the attached questionnaire, please contact the undersigned at (301) 663-8206, Extension 103.
