P14PS01165 PMIS:
Experience Form
Relevant Prior Experience READ CAREFULLY: The Offeror shall address experience of its firm (and/or subcontractors proposed for use on this contract) in performing work described in the solicitation Section L. Offerors must understanding the project as per project schedule and narrative demonstrating sequence of work and critical path of the project and should specifically include any prior experience in: (1) Architectural Concrete Formwork with integrated color and intricate surface finishes installed to close slope tolerances; (2) Asphalt Paving of accessible paths and integration of new work to existing; (3) Working with heavy equipment in an ecologically sensitive environment; (4) Project compliance with Federal Accessibility Regulations; (5) Preparing and implementing site specific safety plans; and
(6) Working in a busy residential pedestrian public right of way, accommodating temporary universal access. The number of projects listed shall not exceed five (5) and shall be numbered consecutively, beginning with the most recent project. Use one sheet for each individual project. Fill in the information by typing or printing legibly.
For experience related to subcontractors, provide the name of the subcontractor and the proposed work to be performed by that subcontractor in the space indicated. Additional narrative may be provided on blank continuation pages if the space provided below is insufficient to allow for a thorough and complete presentation of pertinent information.
