About this file

This document is an Amendment (Amendment No. 6) to a Mandatory Use Contract issued by the State of Ohio's Department of Administrative Services (DAS), General Services Division, Office of Procurement Services. The contract involves Mead Johnson & Company, LLC as the contractor and is specifically for the Ohio Department of Health's Special Supplemental Nutrition Program for Women, Infants, and Children (WIC), focusing on infant formula rebates.

The original contract (No. OT900622) was effective from 10/01/21 to 09/30/24, with the current amendment extending the contract through 09/30/27. The contract allows the Ohio Department of Health to purchase infant formula, with the state reserving the right to renew the contract for up to 36 months total. The amendment maintains all previous pricing, terms, and conditions, and simply extends the contract duration by an additional 12 months through mutual agreement between the State of Ohio and the contractor. The contract is available online at http://www.ohio.gov/procure and provides agencies the flexibility to purchase supplies as needed without guaranteed volume commitments.
