About this file
This document is Amendment No. 4 to a state contract issued by the Ohio Department of Administrative Services (DAS), specifically for the Ohio Department of Health's Special Supplemental Nutrition Program for Women, Infants, and Children (WIC). The amendment involves Mead Johnson & Company, LLC, located in Evansville, Indiana, as the primary contractor for infant formula rebates.
The amendment updates pricing for powdered infant formula, effective March 1, 2025, with specific changes to the Enfamil Infant product line across three physical forms: powdered, liquid concentrate, and ready-to-feed. The pricing schedule details include lowest wholesale full truckload prices, rebate amounts per unit, and percentage rebates. The contract also includes a contract renewal, as noted in a previous amendment (Amendment No. 3), which extended the contract for an additional twelve months from October 1, 2024, through September 30, 2025. The amendment maintains all other existing contract terms and conditions while only adjusting the pricing for the specified infant formula products.
