About this file
This document is an Affirmation and Disclosure Form completed by Drivewyze Ltd., addressing contract requirements for state service provisions, specifically related to Executive Order 2022-02D. The form involves the Ohio Department of Transportation (ODOT) and outlines the contractor's commitments regarding service locations and data management. The company's principal business locations are in Madison, Wisconsin and Albany, New York, with no subcontractors listed.
The form affirms that Drivewyze Ltd. will perform all contract services within the United States, using AWS Cloud Services in US locations (specifically Seattle, Washington), and will not allow state data to be accessed or stored outside the United States without a signed waiver. The document explicitly prohibits service performance in Russia, in compliance with state executive orders, and requires immediate disclosure of any potential changes in service locations. The form is signed by William Decaire, General Counsel, on October 8, 2025, and serves as a binding commitment to the contractual terms and geographic service restrictions.
