About this file
This document is an Affirmation and Disclosure Form for the Ohio Department of Transportation (ODOT), completed by Berry, Dunn, McNeil & Parker, LLC, a consulting firm located in Portland, Maine. The form addresses compliance with Executive Order 2022-02D, which prohibits state agencies from purchasing services from Russian companies or institutions, and reaffirms restrictions on offshore service performance outside the United States. The contractor must disclose all locations where services will be performed and state data will be located, with the understanding that any changes must be immediately reported to the state.
The document indicates that all services for this contract will be performed at Berry, Dunn, McNeil & Parker's primary business location at 2211 Congress Street in Portland, Maine. No subcontractors are listed, and the company affirms that no services will be performed in Russia. The form was signed by Charles Leadbetter, a Principal at the firm, on April 2, 2024, and serves as a binding commitment to comply with state procurement regulations regarding service location and data management. While specific contract details are not provided, the form suggests this is a professional services contract subject to strict geographical and operational requirements.
