About this file

This is a Northern Ireland Fair Employment Practices Disclosure form required by the City of Cleveland for all contractors and subcontractors with more than a fifty percent (50%) interest in proposed contracts. The disclosure must be completed and submitted with the bid or prior to contract award; failure to submit renders the bidder ineligible for contract award. The form requires contractors to declare whether they or any controlling shareholder, subsidiary, or parent corporation conduct business or trading for profit in Northern Ireland. If no such business activity exists, the contractor may proceed directly to signature. If business activity in Northern Ireland is confirmed, the contractor must either certify that they are taking lawful and good faith steps to engage in fair employment practices consistent with the MacBride Principles for Fair Employment in Northern Ireland or provide supporting documentation demonstrating compliance with these standards.

The form enforces compliance with Cleveland Codified Ordinance Section 181.36 and carries significant penalties for non-compliance. Any contractor or subcontractor making a false statement is deemed in default of contract and subject to contractual remedies for default. Failure to cure a default results in automatic exclusion from bidding on any goods or services for the City of Cleveland for a period of two years. The disclosure applies to the REC-N-TEC Pilot Initiative, a workforce development program in Ohio designed to provide digital skills training through the City of Cleveland's Neighborhood Resource and Recreation Center to vulnerable populations including unsheltered residents, seniors, single parents, and justice-affected individuals.
