SF 1449 Block 15 – Deliver To: All requirements herein shall be delivered to the following address FOB Destination:

SF 1449 Blocks 19–24 – Schedule of Supply/Services: The contractor shall provide the labor, equipment, and materials necessary to remove the existing Yale 20-ton electric monorail hoist, procure and complete installation of a new 10-ton electric monorail hoist including all associated mechanical and electrical work at the John C. Stennis Space Center, Mississippi (SSC) in accordance with the Statement of Work (SOW) at Attachment 1. The Government anticipates the award of a Firm-Fixed Price (FFP) contract for this requirement. Offerors shall complete the Price Schedule at Attachment 5 as payment will be made in accordance with the Price Schedule.
SF1449 Block 25 – Accounting and Appropriation Data: To be completed upon contract award.
Period of Performance: The total period of performance for this requirement shall be 180 days after contract award.
Contract Completion: This contract shall be considered complete when all items have been delivered and accepted by designated SSC personnel.

(d) Comptroller General Examination of Record The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)
(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c) and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—

(2) Evaluation Criteria Submission Requirements –
a. Technical Acceptability: Offerors quote shall include the salient characteristics of the proposed product and demonstrate the degree to which it can satisfy the government’s requirement. The quote shall be clear and concise, and shall include sufficient detail for effective evaluation and substantiation of stated claims. The quote shall not simply affirm, rephrase, or restate the Government's requirements, but rather, provide convincing rationale to address how the offeror’s product meets the requirements. Offerors shall assume that the Government has no prior knowledge of their products, facilities, or experience, and will base its evaluation only on the information presented in the offeror's quote. Elaborate brochures or documentation, detailed artwork, or other superfluous embellishments are unnecessary and are not desired. Offerors are advised that existing product literature may be submitted in lieu of unique technical proposals so long as it conforms to the requirements of the solicitation and contains adequate detail for purposes of evaluation. The information provided shall comprehensively explain how offeror proposes to comply with the applicable requirements. Technical proposals should include, at a minimum, detail adequate to demonstrate that the proposed 10-ton hoist meets the requirements identified in the Statement of Work.
b. Delivery Schedule: Offeror’s quote shall include documentation that clearly demonstrates the offeror’s ability to meet the Government’s delivery schedule.
c. Past Performance: Offerors shall provide Past Performance Information (PPI) that includes, at a minimum, the name(s) and contact information for three (3) of the offeror’s most recent (within three years from the offer due date listed on page 1 in Block 8 of the
SF1449) and relevant (information that has a logical connection to the requirement) references (whether federal, state, or local government or private industry) for the prime contractor, and, if applicable, significant subcontractors. Failure to provide adequate contact information shall result in the reference being considered “neutral.” PPI provided should include verifiable experience in providing products and services comparable to that required by this solicitation. References other than those provided by the offeror may be contacted and their comments considered during the evaluation process. The information submitted may be verified by the Government through discussions with the references provided. While the Government may elect to consider data obtained from other sources, the burden of providing relevant references that the Government can readily contact rests with the Offeror.
d. Safety and Health Plan: Offeror’s quote shall include a detailed discussion of the policies, procedures, and techniques that will be used to ensure the safety and occupational health of Contractor employees and to ensure the safety of all working conditions throughout the performance of the contract. The plan shall similarly address subcontractor employee safety and occupational health for those proposed subcontracts or subcontract effort where the work will be conducted completely or partly on Stennis
Space Center. Reference SOW Appendix E (SCWI-8715-0008), Section 8 (Safety and
Health Provisions) for guidance on the information required in the Safety and Health
Plan.
e. Price: Offeror is to provide a general breakdown of pricing sufficient to determine price reasonableness. Offeror shall identify and document all (if any) assumptions, conditions, and/or exceptions upon which the price is based. Any assumptions, conditions, and/or exceptions considered by the Government to be unacceptable may result in elimination for an award.
(3) Evaluation – This acquisition for a commercial item will be conducted pursuant to the simplified acquisition procedures in FAR Part 13. As such, the best value selection shall comply with the procedures in FAR Part 13.106.
a. The Government intends to award one contract from this solicitation to the offeror who is determined responsible in accordance with the Federal Acquisition Regulation, as supplemented, whose offer conforms to the solicitation requirements and is determined, based on the evaluation factors, to represent the best value to the Government. The following factors shall be used to evaluate offers: Technical Acceptability, Delivery
Schedule, Past Performance and Price.
b. Options. Not applicable to this requirement.
c. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
(4) Financial Information – In order for the Government to make an affirmative responsibility determination, offerors shall provide correspondence from its financial institution indicating offeror’s financial capabilities.
(5) Higher-Level Quality Standard – This procurement is subject to higher-level quality standard ISO 9001 or equivalent quality program that encompasses the provisions of ISO 9001.
Offerors shall submit a copy of its ISO 9001 certificate or documentation of an equivalent quality program that encompasses the provisions of ISO 9001.
(6) Proposal Costs – The Government will not pay any offeror for preparation of its quote.
(7) Site Visit Information – The government will conduct a single organized Site Visit for this requirement. The site visit is scheduled for April 12, 2016 at 9:00 a.m. Central Time. Offerors are highly encouraged to attend this meeting prior to submitting an offer, as it will be the only site visit provided. In order to arrange access to SSC, offerors planning to attend shall notify the contract specialist via email at belfield.c.collymore@nasa.gov no later than April 8, 2016 at
4:00 p.m. Central Time with the company name, names of representatives, number of representatives and citizenship. Offerors are limited to 3 individuals per company. On April 12, 2016, Offerors shall report to the SSC South Visitor’s Reception Center prior to 8:30 a.m.
Central Time. Any questions regarding the site visit must be submitted in writing to belfield.c.collymore@nasa.gov. The site visit sign in document will be posted for viewing by potential contractors.
(8) Questions – Questions regarding the solicitation and/or Statement of Work are encouraged to ensure adequate understanding of the government’s requirement. All questions regarding subject solicitation must be submitted no later than 4:00 p.m. Central Time on April 26, 2016.

The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management
(SAM) Web site accessed through http://www.acquisition.gov . If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (q) of this provision.
(a) Definitions. As used in this provision--
“Economically disadvantaged women-owned small business (EDWOSB) concern” means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13
CFR part 127. It automatically qualifies as a women-owned small business eligible under the
WOSB Program.
“Forced or indentured child labor” means all work or service—
(1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or
(2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties.
“Highest-level owner” means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.
“Immediate owner” means an entity, other than the offeror, that has direct control of the offeror.
Indicators of control include, but are not limited to, one or more of the following: Ownership or interlocking management, identity of interests among family members, shared facilities and equipment, and the common use of employees.

“Service-disabled veteran-owned small business concern”—

(i) One or more socially disadvantaged (as defined at 13 CFR 124.103) and economically disadvantaged (as defined at 13 CFR 124.104) individuals who are citizens of the United States; and
(ii) Each individual claiming economic disadvantage has a net worth not exceeding $750,000 after taking into account the applicable exclusions set forth at
13 CFR 124.104(c)(2); and
(2) The management and daily business operations of which are controlled (as defined at
13.CFR 124.106) by individuals, who meet the criteria in paragraphs (1)(i) and (ii) of this definition.
“Subsidiary” means an entity in which more than 50 percent of the entity is owned—
(1) Directly by a parent corporation; or
(2) Through another subsidiary of a parent corporation.
“Veteran-owned small business concern” means a small business concern—
(1) Not less than 51 percent of which is owned by one or more veterans(as defined at 38
U.S.C. 101(2)) or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more veterans; and
(2) The management and daily business operations of which are controlled by one or more veterans.
“Women-owned business concern” means a concern which is at least 51 percent owned by one or more women; or in the case of any publicly owned business, at least 51 percent of the its stock is owned by one or more women; and whose management and daily business operations are controlled by one or more women.
“Women-owned small business concern” means a small business concern --
(1) That is at least 51 percent owned by one or more women or, in the case of any publicly owned business, at least 51 percent of the stock of which is owned by one or more women; and
(2) Whose management and daily business operations are controlled by one or more women.
“Women-owned small business (WOSB) concern eligible under the WOSB Program (in accordance with 13 CFR part 127),” means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States.

(2) The offeror has completed the annual representations and certifications electronically via the SAM website accessed through https://www.acquisition.gov. After reviewing the
SAM database information, the offeror verifies by submission of this offer that the representation and certifications currently posted electronically at FAR 52.212-3, Offeror
Representations and Certifications—Commercial Items, have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation
(including the business size standard applicable to the NAICS code referenced for this solicitation), as of the date of this offer and are incorporated in this offer by reference
(see FAR 4.1201), except for paragraphs ____________.
