ATTACHMENT 5 - QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)
The Government reserves the right to modify any performance standards and/or metrics during the life of this contract, in order to ensure that the appropriate outcomes are being assessed and that the performance standards are appropriate. Changes will be accomplished via a unilateral contract modification.
The QASP for this requirement is provided in the following worksheet. Using the QASP, the bonus or penalty shall be established each month and set between 0% and 10% of the applicable monthly invoice.
The applicable monthly invoice does not include revenue generated from monthly surveys. The maximum bonus that can be received is 10% of the applicable monthly invoice. The maximum percent penalty that can be applied for a given month is 100% of the bonus, or 10% of the applicable monthly invoice, leaving the payment at 90% of the applicable invoice. If all criteria are evaluated to be in the highest Acceptable
Quality Level (AQL), there will be no reduction of bonus or percent penalty applied, and the contractor shall receive the full 10% bonus.
There are nine separate and distinct required services. Each required service has standards that shall be evaluated monthly. Each standard has its own method of surveillance (MS) and AQL. Each AQL was assigned a point value which will be used for a percent reduction of bonus and/or percent penalty associated with it. The AQLs are broken down into distinct ranges, and vary from standard to standard.
At the end of each month, all standards shall be evaluated using the appropriate MS and assigned an
AQL. The applicable percent reduction of bonus and percent penalty is based on the points assigned to the
AQL.
The Division of Radiation Safety (DRS) may establish a Customer Feedback and Analysis (CF&A) system as needed for the QASP § B-2 MS. If established, the CF&A system shall, within a 24 hour period, provide the Contractor with a written copy of each Authorized User’s feedback indicating a violation of Radiation Safety policy occurred. DRS shall cite all violations of Radiation Safety policy submitted through the CF&A system and for each violation cited, state any contractor actions which have contributed to Radiation Safety policy violations. If DRS issues three or more notices that a contractor’s actions have contributed to a violation of the same Radiation Safety policy within any 12 month period, DRS may include the contractor’s actions when establishing the AQL for the month when the most recent notice was issued.
A reduction of bonus and percent penalty are two different enforcement mechanisms. A reduction of bonus that is 50% means 50% of the bonus will be subtracted from the maximum bonus, thus leaving 5% of the monthly invoice as the current bonus. For example, this would apply if the total points deducted were 31-50 based on values listed in the QASP. The percent penalty is a percent of the maximum bonus which is to be subtracted from the monthly invoice. A percent penalty only comes into effect once 100% reduction of bonus has been achieved. Once the 100% reduction of bonus has been achieved, any AQL after that point which is less than the maximum proficiency shall be marked as a percent penalty. A percent penalty of 50% means 50% percent of the maximum bonus will be subtracted from the invoice, leaving the total payment rendered for that month at 95% of the invoice. This would be accredited to a total or 111-120 points deducted based on the criteria listed in the QASP.
