About this file

This is a Request for Services (RFS) form for Wisconsin State Contract #505ENT-M24-TEMPHELPSV-00, which governs non-IT temporary help services. The document serves as a standardized template that Wisconsin state agencies, campuses, and municipalities use to request temporary staffing services from approved contractors under the statewide contract. The form facilitates communication between authorized users (state entities) and contractors, with oversight provided by the State Bureau of Procurement and contract management by Kristina Harris.

The contract covers temporary staffing services across various job classifications, with contractors required to provide qualified candidates based on specific project requirements including duties, skills, certifications, work schedules, and assignment locations. The form captures essential project details such as start/end dates, number of staff needed, reporting structure, and security requirements. Pricing is structured around regular and overtime hourly rates for each position, with invoicing directed to authorized user-specified addresses. The process includes background checks through the Wisconsin Department of Justice for all candidates, optional drug testing, and a feedback mechanism for contract performance evaluation. Contractors must respond to requests within specified timeframes, and authorized users commit to notifying contractors of hiring decisions within 20 business days of the response deadline.
