Scanning system
Dental cone beam computed tomography systems. The systems shall be capable of emitting x-rays from a cone-shaped radiologic source to produce 3D images of the head/neck region, by means of dedicated software algorithms. The systems shall also be capable of panoramic dental imaging. The systems shall be capable of cephalometric imaging, to provide accurate measurements of the head for treatment planning. The systems shall be capable of dental imaging for specialty procedures including those in the specialties of endodontics, implantations, orthodontics, prosthodontics, oral surgeries, and maxillofacial surgeries. ADMINISTRATIVE INFORMATION
CONTRACTING OFFICER’S REPRESENTATIVE (COR):
The Contracting Officer's duly authorized representative will perform inspection and acceptance of supplies/services to be provided.
The COR will be identified in each delivery order.
Inspection and acceptance will be performed at the assigned location according to the performance work statement.
THIS IS A SEVERABLE SERVICE REQUIREMENT AND SPECIAL REPORTING REQUIREMENTS EXIST UNDER 10 USC 2410a AND IN ACCORDANCE WITH FAR 32.703-3.
Contractor employees shall identify themselves as contractor personnel by introducing themselves or being introduced as contractor personnel and displaying distinguishing badges or other visible identification for meetings with Government personnel. In addition, contractor personnel shall appropriately identify themselves as contractor employees in telephone conversations and in formal and informal written correspondence.
CONTRACTOR SUPPORT
NOTICE: NAVY USE OF SUPPORT CONTRACTOR FOR CONTRACT CLOSEOUT FUNCTIONS
Naval Medical Logistics Command uses two private contractors in support of the contract closeout process. Those companies are Contracting Resources Group (CRG) of Federal Hill, MD, doing business under the authority of the Small Business Administration’s 8(a) program, and the Ability One Program, doing business under the authority of the Javits-Wagner O’Day Act(41 U.S.C. § 47).
The contract closeout process includes activities such as processing deobligation modifications, obtaining contractor and requiring activity concurrence, preparing the DD-Form 1594 (Contract Completion Statement), and preparing closed files for archiving. Support contractors may perform additional administrative duties, including filing and processing simple administrative modifications. Performing these functions require the contractor employees to have access to the contract file. Therefore, information you provide to the Government or information already in the possession of the Government may be viewed by these support contractors in the course of performing contract close-out functions. The information that may be made available to the contractor may include pricing and technical proposals and performance information.
Naval Medical Logistics Command has signed Non-Disclosure Agreements with each support contractor employee and has required both contractors to provide a Conflict of interest Mitigation Plan to ensure these employees are firewalled from all business development activity.
By submission of its proposal that was accepted by the Government, the contractor and its subcontractors consent to access of their business sensitive/confidential or proprietary data by the Government’s support contractor personnel in order to perform close out services.

DENTAL DIGITAL IMAGING SYSTEM INTEGRATION AND MAINTENANCE
1.0 INTRODUCTION
1.1 Overview. The purpose of this contract is to provide the Dental Treatment Facilities and Medical Treatment Facilities (hereafter referred to as DTF/MTF) and associated clinics with Dental Digital Imaging (DDI) systems in support of military beneficiaries to include all active duty, reservist, retiree, and dependent populations.
1.2 Scope.
The implementation of the Dental Digital Imaging (DDI) systems are required to incorporate advances in technology, communication standards and practice management for Naval Dental Facilities and Navy and Marine Corps Operational Forces The contractor shall provide all aspects required to successfully install the systems to include all applicable hardware, software, warranties, associated project plan, installation plan, shelving, training, and post warranty service maintenance.
1.3 Type of Contract. This will be an Indefinite Delivery/Indefinite Quantity (IDIQ) contract. The Government anticipates making multiple awards, but reserves the right to make a single award based on the results of the technical evaluation. The contract will establish the overall terms and conditions, but supplies and services will be delivered by delivery orders issued under the contract. In the event of a conflict between a delivery order and the basic contract, the basic contract governs.
2.0 GENERAL REQUIREMENTS
The successful vendor(s) shall provide a system that meets or exceeds clinical requirements and performance and provide the best value. Proposals shall meet the requirements set by the Bureau of Medicine and Surgery, Naval Medical Logistics Command and, the local Commands, and agree to fixed pricing for products and services throughout the multi-year life of the system, subject to the availability of Government funding.
A technical and clinical requirements document will be provided for each order to help the vendor determine the make/model, quantities, and capabilities of the DDI equipment requirements. The DDI system shall be designed to allow Military Navy Dentistry flexibility and provide Dental Imaging solution for the Enterprise. The individual facilities will have a localized DDI system that will capture, save, view, manipulate, print, store and distribute Digital Imaging and Communications in Medicine (DICOM) images within that facility/clinic. A copy of the image will be sent to a second server which is located at that clinics headquarters datacenter or Regional datacenter which acts as an active repository for patient care within the Dental Treatment facilities area of responsibility. The Command regional server shall be capable of sending, updating, viewing, and/or retrieving DICOM and /or integrating the Healthcare Enterprise (IHE) formatted images to and from the Enterprise Clinical Imaging Archive (ECIA). The ECIA shall ultimately be the long term or permanent archive for all of Navy’s medical and dental radiographic images.
2.1 Hardware Requirements
The DDI network shall utilize COTS hardware (server, storage, switches, etc.) consistent with the command existing network hardware. The vendor shall coordinate specific equipment requirements and security settings with NMLC (or designated Service Representative) and the Command’s Chief Information Officer or designated representative. The server configuration shall be based on the current network infrastructure and workload. The capture workstation shall have the capacity to house four years’ worth of patient data and images and be configured to maximize data integrity and redundancy.
The capture computer in the x-ray rooms shall be able to accommodate full size peripheral component interconnect PCI cards. The workstation in the x-ray room may require two network interface cards (NIC’s). Some DDI projects will require computer workstations to have an uninterruptible power supply UPS in every dental treatment room, x-ray room, front desk area, and provider’s offices. Be advised that most dental facilities will utilize government furnished computer workstations.
2.1.1 Hardware Essential Characteristics: The minimum essential characteristics needed for the DDI hardware are as follows:

The regional servers shall meet the minimum requirements as follows:

The systems shall be capable of providing anatomically-correct 3D images utilizing small, medium, and large fields of view (FOV). Small FOV resolution shall be no greater than 0.1 mm voxel size, medium FOV resolution shall be no greater than 0.3 mm voxel size, and large FOV resolution shall be no greater than 0.5 mm voxel size. The small FOV shall be able to encompass a section of the jaw or one temporomandibular joint (TMJ).
Upon installation, the system shall be configured to send all images to the local medical/dental Picture Archiving Communications System (PACS). The system shall conform to Digital Imaging and Communication in Medicine (DICOM) standards. The system shall be capable of interfacing with the local medical/dental PACS and be able to accept and provide the users a DICOM Modality Work List (DMWL). The power requirements are 110V 60 Hz, 100V 50Hz, and 220V 50 Hz, and the system shall be compatible with any of these combinations.
Handheld Laser Barcode Scanner shall meet the minimum requirements as follows:
USB computer interface Barcode stand Shall be able to read standard 1D, 2D and GS1 DataBar symbols
The proposed DDI solution shall interface with all existing image acquisition modalities. The vendor shall plan for and implement a fail-safe capability for imaging in the event that the DDI network goes down.
2.2 Software Requirements
Navy Medicine has identified and deployed Apteryx DICOM Viewer (DCV) software package, which includes DCV DataManager (server Application) and DCV Client (government furnished workstation), as the common dental x-ray acquisition and viewing software of choice This software is compatible with most of the digital radiographic sensors and capture devices that are currently on the market. This software allows the Government to utilize existing DDI equipment when available, and select the best proven technology available. It provides maximum flexibility in choosing DDI hardware and allows for seamless integration of existing analog and digital x-ray equipment. The Apteryx DCV DataManager software shall be placed on a server that meets or exceeds the software server requirements and meets the Navy’s Information Assurance (IA) requirements. There shall be enough DCV client licenses for all the dental treatment rooms, x-ray rooms, Dentist’s Offices, Front Desk areas, System Administrator, and any other location that has a genuine need and is authorized to view Dental Digital Images. Those commands that require occlusal images shall receive a computer radiology scanner that uses an assortment of standard size phosphoric plates to capture the image and scan into the enterprise software. Most facilities have this form of digital imaging capability. The software allows digital images from intra-oral cameras and digital cameras to be captured, viewed and stored. In addition, the images and a viewer can be “burned” to a CD for portability. There may be a requirement for intraoral digital sensors in order to capture intraoral dental images.
2.2.1 The brand name software characteristics needed for the DDI system are as follows:

In order for the software to meet the needs of the Government, the following specifics must be met:
DICOM-based dental imaging suite that is scalable to any size organization.
Shall provide a standardized capture, storage, retrieval, and viewing of images across an entire organization.
Shall support at a minimum the following imaging devices utilized in dentistry: Intra-oral sensors to include Carestream/Kodak 6100 sensors; Phosphor Plate Scanners; Planmeca Panoramic/Cephalometric units; Carestream 3D/CAT scan; Tomography; Intra-oral Cameras; Extra-oral Cameras; Endo scopes; and Flatbed Scanners.
Shall be DICOM and HIPAA compliant with the ability to control who can access the software and what they are able to perform within the application.
System shall integrate seamlessly with the existing Apteryx DVC Data Manager and Clients at each Navy Dental Clinic.
The software shall have full audit trail capabilities which will record the actions of all users.
System shall be able to act as a standalone enterprise dental imaging PACS solution or also be used as the acquisition modality with auto submission to a preexisting PACS.
3.0 SERVICE REQUIREMENTS
3.1 Overview
In addition to providing the required DDI hardware, software, and associated tasks listed above, the successful offeror(s) will also provide a project plan, installation plan, installation documentation to include a Bill of Materials (BOM) with line item pricing and services, training plan, system configuration and start-up supplies, warranty and maintenance plan to include an online help desk, in order to meet the full scope of this requirement.
3.2 Project Plan.
The DDI installation shall include a complete project plan of action and milestones including a schedule, listing of all tasks and an anticipated timeline for equipment installation and acceptance testing. The proposal shall identify a project manager and a site project team to work pre-installation activities and hold regular planning and status update teleconferences with the government project team. The vendor’s project manager(s) will fully participate in regular status update briefings to the government’s representatives. The vendor’s project team will engage the NMLC Program Office, Command’s Project Coordinator, Dental Clinical, Information Management Department (IMD), Materials Management and Facilities leads to ensure that all devices are properly accounted for, as needed. See paragraph 6 for additional information.
3.3 Implementation Plan.
The implementation plan will ensure minimal disruption to clinical operations of the facility. Provide a detailed Bill of Materials (BOM for each order broken down by facility). Copies of all documents will be provided in soft copy using standard Microsoft products, such as MSProject, MSWord, Excel Spreadsheets, Visio and PowerPoint as appropriate.
3.4 Training Plan
The vendor shall offer training that begins with an initial introduction to the software application, proper intra oral sensor placement techniques, and digital panographic unit (if required) to help first time users to become familiar with the system. Simply having initial training after installation of the system is not enough. Training programs shall include a mechanism that addresses recurring/refresher training to facilitate staff's understanding of the platform updates, maintaining system competency levels, and meeting accreditation requirements. Training shall incorporate the perspectives of the dental X-Ray Technician, the general clinical user, the specialist, and the system administrator. These perspectives consider the varying levels of detail and application interaction that may be required by the dynamic groups of users that will typically utilize an enterprise DDI solution.
3.5 Warranty
The vendor shall provide full warranty support of all new hardware and software, supplies and services for a period of not less than 1 year. Vendors shall acknowledge that there will be no charge for services or software support during the warranty period (for normal warranty support). Additional details regarding the Warranty requirements are located in Section 7.
3.6 Maintenance Plan
A maintenance plan to support the DDI equipment shall be proposed for a period of up to 4 additional years following warranty expiration as option periods on the resultant orders (will vary based on the performance year of the refresh and base contract expiration).
4. BASIC OPERATING PRECEPTS
The following operating precepts will apply to the selected DDI configuration;
· Seamlessly acquire, view, print, store, and transfer images and data within the dental clinic, the Command area of responsibility, other Navy Commands, and Army and Air Force Dental Treatment Facilities.
· Conduct a pre-deployment site survey to validate site specific requirements and update the project plan.
· Provide a redundant system configuration for data integrity and minimize system down time.
· Image communication into and out of the DDI application must be in DICOM version 3.0 or higher format.
· Conduct training on the new system for existing staff and incorporate an enterprise wide training program for newly arrived personnel either by Computer Based Training (CBT) or Web Based training. Provide “train the trainer” education for selected staff members so they may provide future training.
5. DDI CORE COMPONENT REQUIREMENTS
5.1.1 DDI Client Application
The offeror shall ensure only authorized dental care providers have the capability for a secure easy way to capture, save, view, manipulate, print, store and retrieve diagnostic quality digital dental images.
5.1.2 Clinic Level DDI Server
The offeror shall provide a DDI server with the short-term storage capacity at the capture workstation and/ or at the local DDI server for no less than 3-5 years of current patient images as a buffer for the archive and to provide a fail-safe measure in the event the archive is not available. Image retrieval time from short-term storage shall be consistent with the industry standards. This server shall be setup to forward all studies to the Command-wide DDI server and the Enterprise Clinical Imaging Archive (ECIA). The minimum storage capacities for each facility/clinic will be identified in the delivery order.
5.1.3 Command-Wide (Regional) DDI Server
A larger capacity DDI server capacity shall have no less than 3-5 years of current patient images for the entire Command or region with the ability to view and maintain all archived images in an “online” status. The vendor shall assist in determining the specific required storage capacity for each facility based on the current and anticipated workload. Hot swappable RAID drives with enough storage capacity for a mirrored back-up of the database and images. Vendors shall offer a solution that will last at least 5 years before needing additional archive capacity. Image retrieval time from the archive shall be consistent with the industry standards. The archive shall maintain a permanent record of all imaging studies acquired and transmitted to the network. The vendor shall provide a network configuration with minimum bandwidth requirements to support a regional archiving and enterprise database. The minimum storage capacities for each facility/clinic will be identified in the delivery order.
5.1.4 Image Acquisition Modalities Supported
At a minimum, the following modalities/dental equipment shall be supported;
· Digital Panoramic
· Intraoral x-ray unit
· 3D Cone Beam Computed Tomography (CBCT) units
· Intra-oral sensors
· Computer Digital Scanner
· Intra-oral cameras
· Digital photography
The vendor shall support the integration of all dental imaging modalities, including existing interfaces, and report all requirements necessary to bring each imaging modality into full DICOM compliance.
5.1.5 CD Burning
Most Dental facilities shall require a CD/DVD publishing station to meet their workflow requirement. Specified government furnished workstation shall also be capable to burn DDI images to a CD or DVD. The CD/DVDs produced shall have DICOM images with an intuitive DICOM viewer imbedded. Vendor provided CD/DVD publishers shall be able to encrypt the disc and allow for the patient to input a unique password or pin. The vendor shall provide a startup supply of CD’s, labeling kits, and at least 200 CD holders that can be easily placed in the patient’s medical/dental record.
5.1.6 Image Display Workstations
The color monitor shall display color images and patient administrative data. See minimum specifications mentioned in Section 2.1.1. The workstation software and hardware shall be able to display large image sets along with comparison studies.
5.1.7 Network Printers
A networkable photo quality printer is required at each front desk area at each facility. These printers will meet the short term need to have radiographs in the patient’s record until the full implementation of the DDI system. At least a 100 sheet of premium photo quality paper (8.5” x 11”) per printer shall be included.
5.1.8 Network
As needed, the selected DDI vendor may be required to run wiring, setup the computers, servers and switches, verify and validate connectivity within the unit’s dental department DDI LAN to ensure optimum performance of the proposed DDI solution. All networked equipment connections shall be coordinated through the units Chief Information Officer or their designated representative. Work shall be approved with the COR in advance of commencement.
5.1.9 Training
The vendor shall offer training that begins with an initial introduction to the software application, proper intra oral sensor placement techniques, and digital panographic unit (if required) to help first time users to become familiar with the system. Simply having initial training after installation of the system is not enough. Training programs shall include a mechanism that addresses recurring/refresher training to facilitate staff's understanding of the platform updates, maintaining system competency levels, and meeting accreditation requirements. Training shall incorporate the perspectives of the dental X-Ray Technician, the general clinical user, the specialist, and the system administrator. These perspectives consider the varying levels of detail and application interaction that may be required by the dynamic groups of users that will typically utilize an enterprise DDI solution.
Also, sustainment training that may be computer based shall be addressed and provided.
5.1.10 Project Management
Following award of a delivery order the awardee shall provide a detailed example of their project management plan and schedule for the facility/region. The plan shall identify whether a dedicated on-site project manager is provided and whether they will be there for the duration of the project. The plan shall describe how the offeror plans to transition the DDI equipment causing the least amount of downtime in clinical operations. The plan shall identify the tasks required for a smooth transition.
5.1.11 Warranty
During the 12 month warranty period, the Vendor shall furnish warranty service support that includes, as a minimum, corrective maintenance services for the DDI system and all associated hardware, firmware and software, parts, labor, travel and expenses necessary to perform such services at no additional cost to the Government, except in those circumstances where maintenance or repair service is required as a direct result of abuse, misuse, misconduct or other gross or willful damage done by the Government. The Vendor shall perform corrective maintenance services for Government initiated maintenance calls placed outside the Principal Period of Maintenance (PPM).
In those emergency situations when a core system component(s) is down and the Government places the call for corrective action to the Vendor within the PPM, the Vendor shall respond within the prescribed PPM response time and continue efforts to resolve the problem even though maintenance service may begin outside of the PPM (due to response time lag of the Vendor).
The Government representative will make formal notification of the problem by phone to the Vendor POC as soon as the problem is identified by the Government. Such notification will cause the response time and downtime measurements (if call is placed during the PPM) to commence.
6. ACCEPTANCE TESTING (AT)
AT will be conducted in accordance with the specifications outlined in this contract. AT will be scheduled by NMLC and the Vendor within 30 days of the Contracting Officer’s receipt of the “notice of readiness to inspect.” At a minimum, the vendor Project Manager shall be onsite during the AT.
7. POST-WARRANTY MAINTENANCE
Vendor shall provide a maintenance program for each system installed for up to 4 option years following conclusion of the one year warranty.
During each 12 month option period, the Vendor shall propose firm fixed pricing for maintenance service support that includes, as a minimum, warranty level support, scheduled and corrective maintenance services for the DDI and all associated hardware, firmware and software parts, labor, travel and expenses necessary to perform such services. There shall be no additional cost to the Government except in those circumstances where maintenance or repair service is required as a direct result of abuse, misuse, misconduct or other gross or willful damage done by the Government.
All definitions and requirements as defined above for the warranty period shall apply during the post-warranty period unless exceptions are defined.
8. ONLINE HELP DESK.
Vendors shall provide customers with online access to a computer base help desk.
The Online Help Desk shall permit customers to request scheduled and unscheduled services, query historical data and generate reports.
The Online Help Desk shall provide real time information to include current status, total equipment down time, who initiated the service ticket, the original complaint, response time, the service technician, work performed, replacement parts and completion date and time. This shall be included in the proposed price for maintenance.
Online Help Desk Support shall be included with Post-Warranty Maintenance.
9. PLACE OF PERFORMANCE. See PWS Enclosure 1.
10. SECURITY REQUIREMENTS
In order to gain entry and provide installation, inspection and training services for medical/dental equipment and material at all Naval bases and many other Department of Defense Facilities, contractor and sub-contracted personnel must be U.S. citizens, and unless they currently hold an active U.S. government security clearance that can be verified via the Joint Personnel Adjudication System (JPAS), are required to enroll in the RAPIDGate program at www.Rapidgate.com. Contractor and sub-contractor companies are also required to screen the names of those personnel through the E-Verify system operated by the Department of Homeland Security.

H. Special Contract Requirements
H.1. The government intends this Request for Proposal (RFP) to result in award of two or more indefinite delivery indefinite quantity (IDIQ) contracts. Dental Digital Imaging (DDI) refresh solutions will be procured via the award of Orders issued against the basic contracts. Orders will be awarded on a firm fixed price basis via a Standard Form (SF) 1449 signed by the Contracting Officer. Orders will be executed in writing by the Contracting Officer and transmitted via e-mail.
H.2. The contractor shall furnish DDI refresh and maintenance in accordance with the Performance Work Statement, individual orders for the DDI refresh and maintenance, and all other terms and conditions set forth herein. Government requirements for DDI refresh and maintenance shall be filled in response to Orders issued by the Government against the base contract.
H.3. The following activity is the sole authority to issue Orders:

The procuring contracting office is responsible for issuing and administering any order placed against the basic contract. Ordering officers have no authority to modify any provision of the basic contract. Any deviations from the terms of the basic contract must be submitted and approved by the Contracting Officer for contractual action.
H.4. Locations. Shipping will occur to CONUS and OCONUS locations. Installation and training may be required at CONUS and OCONUS locations. See Performance Work Statement -Enclosure 1 for a list of all locations.
H.5. Each Order will contain at a minimum the following information:
· Date of Order
· Contract Number and order number
· Contract item number, description, quantity and unit price
· Delivery schedule
· Place of delivery (including co-signee)
· Any packaging, packing and shipping instructions
· Accounting and Appropriation data
· Payment office address
· Invoicing and acceptance instructions
· Name of the COR
· Any other pertinent data
H.6. Maximum Quantities. All available quantities for this contract are given in Contract Line Item Numbers (CLINs) 0001 through 0017 of Section B.
H.6.1. Award of Initial Requirement /Minimum Guarantee Initial requirements will be awarded under task orders after the basic contracts have been awarded. Only the offerors that are selected for a MATO IDIQ contract award will be awarded the initial requirements (Minimum Guarantee). Initial requirements represent all known requirements at the time the Solicitation is released. Initial requirements are identified in the electronic pricing workbook. The Government reserves the right to adjust the initial requirements based on need and/or available funding.
H.7. The estimated ordering period is 60 months, beginning at the start of basic contracts, or until the time the Government has issued Orders totaling the sum of the maximum quantities. The period of performance for the base period or any option periods of a Task Order shall be of one year or less in duration.
H.8. Status of Forces Agreements. Status of Forces Agreements (SOFAs) between the United States and foreign countries govern the rights and obligations of the United States armed forces in those countries.
H.8.1. For work performed in Japan:
H.8.1.1. U.S.-Japan bilateral agreements govern the status of contractors and employees, criminal jurisdiction, and taxation;
H.8.1.2. USFJ (U.S. Forces Japan) and component policy, as well as U.S.-Japan bilateral agreements, govern logistic support and base privileges of contractor employees;
H.8.1.3. The Commander, USFJ, is primarily responsible for interpreting the SOFA and local laws applicable to U.S. Forces in Japan and for requirements in support of USFJ; and
H.8.1.4. A copy of the SOFA is available at the following website: https://www.state.gov/documents/organization/108914.pdf.
H.8.2. For work performed in Korea:
H.8.2.1. U.S.-Korea bilateral agreements govern the status of contractors and employees, criminal jurisdiction, and taxation;
H.8.2.2. USFK (U.S. Forces Korea) and component policy, as well as U.S.-Korea bilateral agreements, govern logistics support and base privileges of contractor employees;
H.8.2.3. The Commander, USFK, is primarily responsible for interpreting the SOFA and local laws applicable to U.S.
