Attachment 14 Additional Technical Proposal Instructions
Surveillance, Epidemiology, and End Results (SEER) Program
MANDATORY QUALIFICATION CRITERIA It is imperative that Offeror read and prepare for submission of MANDATORY QUALIFICATION CRITERIA in Section M. Evaluation Factors for Award, Item 4 of the Solicitation. There are absolutely no exceptions to meeting the Mandatory Qualification Criteria at time of Proposal Submission. If the Offeror’s proposal does not meet the mandatory criteria, the technical proposal will not be forwarded to the technical evaluation committee for technical evaluation and therefore the Offeror will not be considered for award. If an Offeror proposes a consortium comprised of groups of counties/parishes within a state(s) or groups of states/territories/federal district, then the total proposed consortium shall meet all of the mandatory criteria.
This and the following Additional Technical Proposal Instructions reflect the requirements of this Request for Proposal and are meant to provide additional instructions as well as uniform format for technical proposals. The information provided and requested in these instructions should be used as a guide for formatting and preparing the technical proposal. This information is provided in addition to instructions provided in Section L of the Solicitation.
A detailed work plan must be submitted indicating how each aspect of the Base, Sample Task Order A, and Sample Task Order B statements of works are to be accomplished. Your technical approach should be in as much detail as you consider necessary to fully explain your proposed technical approach or method.
The technical proposal should reflect a clear understanding of the nature of the work being undertaken.
The technical proposal must include information on how the project is to be organized, staffed, and managed to allow evaluation as provided in Section M - Evaluation Factors for Award of this solicitation.
Information should be provided which will demonstrate your understanding and management of important events or tasks.
The Offeror must submit a detailed explanation of the proposed technical approach in conjunction with the tasks to be performed in achieving the project objectives of the Base SOW and all Sample Task Order SOW’s. Proposals which merely restate the requirements of the Government's scope of work will not be eligible for award.
Offerors are advised to give careful consideration to the Base, Sample Task Order A and Sample Task Order B Statements of Work and all reference material, attachments, the technical evaluation criteria, and the Solicitation as a whole, in the development of their Technical Proposal.
Section L.2.a and L.2.b. of the Solicitation discusses the acceptable minimum requirements for the format and contents. The instructions below should be used, along with Section L. of the Solicitation to format and prepare the Technical Proposal.
Submit one Technical Proposal divided into the Sections listed below. Offerors must adhere to the page limits specified below. Pages that exceed more than the specified limitations for Sections 3, 4, and 5 shall be removed from the proposal and shall not be provided to the reviewers to be read or evaluated. Attachments and Appendices do not count against the page limitations.

GENERAL INSTRUCTIONS:
A. The Offeror’s Technical Proposal encompasses technical discussion of Base, Sample Task Order A, and Sample Task Order B will be scored in accordance with Section M of the Solicitation.
Scored Technical Evaluation Factors include: Technical Approach, Personnel, Organizational Experience and Capabilities, and Resources and Facilities.
B. Non-scored proposal factors include: human subject, mandatory qualification criteria, evaluation of options (Sample Task Orders A and B), demonstration of a strong scientific premise for the technical proposal (Sample Task Order A and Sample Task Order B), authentication of key biological and/or chemical resources, and data sharing plan,). These are also important to the overall contract award decision. All non-scored proposal attributes must be determined to be minimally acceptable. These must be addressed in the Base, Sample Task Order A and Sample Task Order B Technical Proposals. Discussion should be specific to address the respective Statements of Work.
C. All proposal information and documentation required as specified in the Solicitation should be included and listed in the Table of Contents with a page reference.

D. Offerors who propose subcontracts to perform portions of the Base, Sample Task Order A and
Sample Task Order B Statement of Work (SOW) should clearly identify the specific task areas for which they plan to utilize subcontractors, as well as the method and level of integration between the prime and subcontractor(s), and the expected advantages of such an approach. If the proposed approach will involve a subcontracting arrangement, then the Offeror shall include a letter of commitment from the subcontractor(s), plus documentation of subcontractor’s expertise, qualifications, and prior performance, as well as a narrative describing how the Offeror will manage the subcontractor.
E. The Government shall use the Offeror’s Technical Proposal to assess the Technical Approach, Personnel, Organizational Experience and Capabilities and Resources and Facilities. Offerors are apprised that for the evaluation under each technical evaluation factor, the Government shall confine its consideration to the section of an Offeror’s proposal that corresponds to that technical evaluation factor. Therefore, in the technical proposal, the Offeror should clearly mark each section to correspond with each technical evaluation factor, and ensure that all information which the Offeror wants the Government to consider for that technical evaluation factor is within the designated section, or, in the case of any attachments, is clearly referenced as incorporated into the correct section. Failure to do so means that the inappropriately placed information may not be considered under the technical evaluation factor.
F. No hardcopies are due. The proposal shall be submitted electronically using the Electronic Contract Proposal System (eCPS). Refer to Attachment 1 Packaging and Delivery of the Proposal for Use with eCPS for instructions.
G. A Technical Proposal is not required for Sample Task Order C. See the Additional Business Proposal Instructions for directions on preparing a budget for this Sample Task Order.
SPECIFIC INSTRUCTIONS:
The Offeror is responsible for ensuring that all required information and documentation is included in their Technical Proposal in accordance with the Solicitation. The following is additional information and does not include all required information and documentation for the technical proposal.
SECTION 1:
The following does not count toward the page limitations.
a. PROPOSAL TITLE PAGE. Include RFP title and number, name of organization, DUNS number, identification of the proposal part, appropriate titles and signatures of the Business Official and Principal Investigator.
b. PROPOSAL SUMMARY AND DATA RECORD (NIH-2043); Do not include any Social Security Numbers (SSNs) on the form.
c. TABLE OF CONTENTS. Include a Table of Contents (TOC) that provides the page number of each proposal section that at a minimum uses the headings and subheadings that appear in the chart above. Additional subheadings may be entered at the Offeror’s discretion. Refer to Other considerations within Section L of the Solicitation.

SECTION 2:
a. MANDATORY QUALIFICATION CRITERIA (Not Scored) required documentation does not count toward the page limitations.
b. Read the first paragraph of this document and review MANDATORY QUALIFICATION CRITERIA in Section M. Evaluation Factors for Award, Item 4 of the Solicitation.
SECTIONS 3-5:
The Offeror should include the Technical Discussions in the relevant section of the Technical Proposal and ensure that the Offeror’s Technical Proposal addresses all required information and documentation in accordance with Section L.2.b. Technical Proposal Instructions of the Solicitation.
The following pertains to SECTIONS 3 (Sample Task Order A SOW); SECTION 4 (Base SOW Task Areas 2, 4, 5, 6, 7, and 8); and SECTION 5 (Sample Task Order B SOW).
The Offeror must separate Technical Discussions for each SOW as per the chart above. Refer to Section M. for Technical Evaluation Factors. The following factor and sub-factor weights correspond to Section M.

a. Technical Approach (45%):
If Subcontractors are proposed the Subcontractor’s Technical Proposals must include the information below and include the required documentation:
1. Discuss the Offeror’s understanding of the technical, scientific and operational problems and solutions associated with accomplishing the requirements of all tasks in the SOW.
2. Discuss the Offeror’s understanding of the issues and problems of collecting population level cancer surveillance data associated with accomplishing the requirements of the SOW.
3. Describe the Offeror’s managerial approach to accomplishing the requirements of the SOW.
4. Describe the Offeror’s management approach to supervising and managing contractor personnel, subcontractor personnel and consultants proposed to conduct the SOW.
5. Describe the Offeror’s management approach to staffing, training and the efficient use of staff or other resources to conduct the SOW. Reference within your technical discussions and include 3 separate Staffing Plans under Attachment and Appendices of Sections 3, 4, and 5.
Each Staffing Plan must be specific to one if the three technical discussions (Base Technical Discussion, Sample Task Order A Technical Discussion and Sample Task Order B Technical Discussion)
6. Describe in detail the Offeror’s staffing plan delineating lines of authority and areas of responsibility associated with accomplishing the requirements of the SOW. Include staffing plan in Attachment section.
7. Describe the Offeror’s proposed methods for ensuring quality assurance and adhering to milestones and timely submission of reports and deliverables.
8. Describe the Offeror’s proposed quality control and quality assurance procedures to ensure the requirements of the SOW are met.
9. Describe the Offeror’s plan to ensure confidentially of information.
10. Describe the Offeror’s methods for ensuring the prevention of Conflicts of Interest for the
SOW.
11. If Subcontractors are proposed, provide the Prime’s Technical Proposal should briefly discuss the Subcontractor’s technical approach and refer to the Subcontractor’s Technical Proposal for detailed discussion. The Subcontractor’s Technical Proposal should be included as an attachment to the Prime Technical Proposal. The Subcontractor’s Technical Proposal should describe and discuss 1 - 10 above and include signed subcontractor commitment letter from each organization. Include the subcontractor commitment letters under Attachments and Appendices of Section 3.
b. Organizational Experience, Capabilities, Resources and Facilities (30%):
If Subcontractors are proposed the Subcontractor’s Technical Proposals must include the information below and include the required documentation:
a. Describe organizational experience and ability to carry out the specific tasks and requirements of the SOW including the organization’s experience that:
i. supported central cancer registry operations
ii. supported research which directly utilized the central cancer registry's data collection infrastructure; case control studies, patterns of care, comparative effectiveness or patient outcomes, or studies requiring tissue acquisition
iii. established and maintained data sharing agreements with other population-based cancer registries and other entities
iv. established relationships with the Offeror's local cancer research and cancer control communities and collaborated with other organizations

v. supported the release of the Offeror's cancer surveillance data to outside investigators
vi. worked with Institutional Review Boards
vii. provided and maintained an available, adequate and a secure data processing environment including information security and physically secure office space including:
1. security of computer network systems and support
2. physically secure office space
3. data and physical security
4. office equipment
5. administrative support
b. Describe availability and adequacy of the facilities to perform the tasks of the SOW including administrative support and dedicated space for personnel and equipment.
c. Describe availability and adequacy of the secure environment for confidential storage of information including physically secure office space, data, and physical security.
d. Describe availability and adequacy of the equipment to perform the tasks of the SOW including all computers, hardware, software, and servers.
e. If subcontractors are proposed, describe and discuss b, c, and d. above.
f. Describe and document availability of qualified personnel to conduct the SOW.
g. Describe organizational experience for assuring quality of work, products and deliverables and capability to conduct quality assurance.
h. Describe organizational experience and capability of maintaining stable workforce and ability to recruit and retain staff.
i. Describe organizational experience in appropriate use of and managing subcontractors to supplement areas of expertise to carry out the tasks of the Base SOW.
j. If subcontractors are proposed, the Prime’s Technical Proposal should briefly discuss the Subcontractor’s organizational experience, capabilities, resources and facilities and refer to the Subcontractor’s Technical Proposal for detailed discussion. The Subcontractor’s Technical Proposal should describe and discuss a - i above.
c. Personnel (25%)
If subcontractors are proposed, the Prime’s Technical Proposal should briefly discuss the Subcontractor’s proposed personnel and refer to the Subcontractor’s Technical Proposal for detailed discussion.
1. Describe and document the education, training, experience and expertise of the proposed key personnel required to carry out the tasks of the SOW. If Subcontractors are proposed their Technical Proposals must include the information below and include the required documentation as an attachment to the Subcontractor’s Technical Proposal:
a) Key Personnel (Principal Investigator (PI)/Program Director):
i. Describe and document evidence of education, training, expertise, and experience of Principal Investigator/ Program Director to:
1. perform the tasks described in the SOW
2. in cancer epidemiology and conducting population-based epidemiological studies including supervisory and budget experience
3. manage the overall SOW

4. in procedural aspects of managing and coordinating population-based cancer registry data collection including supervisory and budget experience
5. include Resume, CV or NIH Biosketch for each named key personnel in the relevant Attachments section
6. Refer to the following in the Solicitation: Article G.2. Key Personnel, Article H.11 Multiple Principal Investigators, and Section L.2.b. Also, refer to Technical Proposal Instructions for discussion of personnel
b) Key Personnel (Registry Operations Supervisor/Registry Director/Registry Manager):
i. Describe and document evidence of education, training, expertise, knowledge, and experience of Registry Operations Supervisor/Registry Director/Registry Manager to manage the requirements of the SOW that pertain to registry operations.
ii. Describe and document evidence of education, training, expertise, knowledge and experience of Registry Operations Supervisor/Registry Director/Registry Manager in all procedural aspects of managing and coordinating population-based cancer registry data collection including supervisory and budget experience.
iii. Include Resume, CV or NIH Biosketch for each named key personnel in the relevant Attachments section. Refer to the following in the Solicitation:
Article G.2. Key Personnel, Article H.11 Multiple Principal Investigators, and Section L.2.b. Also, refer to Technical Proposal Instructions for discussion of personnel.
c) Other (Non-Key) Personnel:
i. Describe and document evidence of education, training, expertise and experience of proposed personnel to carry out the tasks of the SOW, including specific area of expertise to be contributed to the project. Include:
1. proposed contractor personnel
2. proposed subcontractor personnel
3. proposed consultants
ii. Submit one resume, CV or NIH Biosketch for each named personnel including proposed subcontractor personnel and proposed consultants Refer to Technical Proposal Instructions for discussion of personnel
iii. If To Be Determined (TBD) Positions are proposed, include a discussion of the qualifications and requirements of the position, a description of the “ideal” candidate, the plan for recruitment, and an estimate of the time required to fill the position

SECTION 6:
a. Human Subjects Discussion (Non-Scored)
i. The Offeror should provide a thorough and complete description of the procedures, policies, and systems that demonstrate how the Offeror is going to assure the protection of human subjects with respect to collecting and maintaining data that are subject to provisions of the Confidentiality of Information clause, the Privacy Act, and the Health Insurance Portability and Accountability Act. Additionally, describe any plans that may derive from statutory requirements regarding the notification of patients that their data are in a state cancer registry as well as any plans and activities to educate and inform the public about the existence of cancer registries and their role in reducing the cancer burden.
ii. Include a discussion of protection of human subjects, required education in the protection of human subjects’ research participant’s discussion and data collection analysis plan.
Refer to specific information provided in Section L.2.b.4 of the Solicitation and address the subparagraphs within.
b. Authentication of Key Biological and/or Chemical Resources Discussion (Non-Scored)
The Offeror should review Section M.5 f the RFP to determine whether this topic is relevant to this RFP. If not, the Offeror should merely make a statement that this area does not apply with a brief description as to why.
c. Demonstration of a Strong Scientific Premise for the Technical Proposal Discussion (Non- Scored)
The Offeror should review Section M.9 of the RFP and provide a brief discussion of how; if the aims of the Base, Sample Task Order A, and Sample Task Order B Statements of Work as achieved, scientific knowledge, technical capability, and/or clinical practice would be improved.
d. Data Sharing Plan (Non-Scored)
The Offeror should describe their Data Sharing Plan. This plan should document the Offeror’s plans for directly sharing the data collected not the public access of the SEER data provided by the NCI. At the minimum the plan should address:
i. what data will be shared

The Offeror should provide the following information on population characteristics and special registry features. i. Population Characteristics
When providing race/ethic population information the Offeror should use the definitions that are used by the Bureau of the Census in the Official 2010 U.S.

GENERAL INSTRUCTIONS
1. Submit one business proposal divided into the sections in the chart above.
2. All proposal information and documentation required as specified in the Solicitation should be included and listed in the Table of Contents with a page reference.
3. There are no page limitations to the business proposal or any sections of the business proposal.
4. ALL OFFERORS ARE REQUIRED TO SUBMIT COST PROPOSALS FOR SAMPLE TASK
ORDERS A, B, AND C. DO NOT SUBMIT A COST PROPOSALFOR THE BASE
STATEMENT OF WORK.
5. The Business proposal shall be submitted electronically using the Electronic Contract Proposal System (eCPS). Refer to Attachment 1 Packaging and Delivery of the Proposal for Use with eCPS for instructions. No hardcopies are to be submitted.
6. Section L of the Request for Proposal (RFP) specifies the minimum documentation requirements for cost data and all cost related support. All related documentation should be included in the business proposal in a clearly marked section. Refer to detailed instructions below for Sample Task Orders A, B, and C.

SPECIFIC INSTRUCTIONS:
The Offeror is responsible for ensuring that all required information and documentation is included in their Business Proposal in accordance with the Solicitation in Section L and as specified in Section J, List of Attachments. The following is additional information and does not include all required information and documentation for the business proposal.
SECTION 1:
1. BUSINESS PROPOSAL TITLE PAGE.
The following must be provided on the first page of the business proposal:
a. Solicitation title and number;
b. Name and address Offeror;
c. Offeror’s DUNS number;
d. Identification of the Proposal Part (Business);
e. Name and telephone number of Business Official;
f. Name of contract administration office (if available);
g. Type of contract action (New contract)
h. Proposed cost; profit or fee; and total; Note: Specific for Sample Task Order A, B, and C.
i. Indicate whether you require the use of Government Property in the performance of the contract and if so, what property;
j. Whether your organization is subject to cost accounting standards; whether your organization has submitted a CASB Disclosure Statement, and if it has been determined adequate; whether you have been notified that you are or may be in noncompliance with your Disclosure Statement or CAS, and, if yes, an explanation; whether any aspect of this proposal is inconsistent with your disclosed practices or applicable CAS, and, if so, an explanation; and whether the proposal is consistent with your established estimating and accounting principles and procedures and FAR Part 31, Cost Principles, and, if not, an explanation;
k. The following statement: This proposal reflects our estimates and/or actual costs as of this date and conforms with the instructions in FAR 15.403 5(b)(1) and Table 15 2. By submitting this proposal, we grant the Contracting Officer and authorized representative(s) the right to examine, at any time before award, those records, which include books, documents, accounting procedures and practices, and other data, regardless of type and form or whether such supporting information is specifically referenced or included in the proposal as the basis for pricing, that will permit an adequate evaluation of the proposed price;
l. Date of Submission; and
m. Name, title and signature of authorized representative.
2. PROPOSAL SUMMARY AND DATA RECORD (NIH-2043); Do not include any Social Security
Numbers (SSNs) on the form.
3. TABLE OF CONTENTS. Include a Table of Contents (TOC) that provides the page number of each proposal section that at a minimum uses the headings and subheadings that appear in the chart above.
Additional subheadings may be entered at the Offeror’s discretion. Refer to Other considerations within Section L of the Solicitation.

1. The Offeror should ensure that the business proposal includes all required information and documentation including required attachments in accordance with the Solicitation. The Offeror is strongly encouraged to organize the business proposal in the order listed in the table above.
2. Past Performance Information (Not Scored) In accordance with Section L.1.a.15., provide a list of up to the last 5 contracts or grants completed during the past five years and 1 contract or grant currently being performed that are similar in nature to Base Statement of Work Task Areas 2, 4, 5, 6, 7 and 8. Include the following information for each contract or subcontract:

3. Information Security Attachment
a. Submit a Draft IT-SP as an Attachment to the Business Proposal.
b. Evaluation of Draft IT-SP: The Draft IT-SP will be reviewed to determine that it meets the requirements set forth in the Solicitation. The draft IT-SP will be evaluated but not scored.
The plan must be acceptable for award to be made. Refer to Section L.1.1.17 Information Security and Physical Access Reporting Requirements and to Attachment 14, Additional Technical Proposal Instructions, Sections 3 – 5.c.vii. Organizational Experience and Capabilities.
4. Property Discussion
a. Refer to L.2.c.10.a. Property and provide required information.
b. Discuss and provide list of contractor property proposed for use under Sample Task Order A and B for which no costs are proposed.
c. Equipment proposed under Cost or Contractor Share for Sample Task Order A would be considered Contractor’s Property not Government Property.
d. Government-Furnished Property if requested will be determined during discussions with those in the Competitive Range. The Offeror should include a list of property that is

5. Annual Financial Report
This applies to the Prime Offeror. If the Offeror does not have an annual financial report or audited financial statement, provide the most recent balance sheet.
6. Adequate Accounting System (required as Cost-Type Task Orders will be solicited under resultant IDIQ)
a. As per Section L.2.c.10.d., include the required certification,
b. Include the response to the question regarding whether your organization’s accounting system has been approved for use on Government cost reimbursement contracts
c. Provide the Contracting Officer Determination Letter or give the name and address of the agency that approved the system.
d. For cost-type subcontracts proposed, include the subcontractor’s certification and information.
7. Qualifications of the Offeror As per L2.c.11, provide a summary of your organization’s “General Experience, Organizational Experience Related to this RFP, Performance History and Pertinent Contracts”.
8. Section K – Representatives, Certifications, and Other Statements of Offerors
Submit Attachment 8 which is a NIH form in addition to completing online representations and certifications on www.sam.gov as indicated in Part IV, Section K of the Solicitation.
Important note: The North American Industry Classification System (NACIS) code used in the Small Business Sources Sought Notice, the Research and Development Sources Sought Notice, and the Draft Request for Proposal was 541712 which is no longer in use. The 2017 NAICS used in the Solicitation is 541715. The Offeror is advised to update their https://www.sam.gov records accordingly. Refer to https://www.census.gov/cgi-bin/sssd/naics/naicsrch?code=541715&search=2017%20NAICS%20Search .
9. Non-Disclosure Agreement for Prospective Offeror
Use the link in Section J for Attachment 9 and submit signed form for each named contractor and subcontractor personnel who may have access to non-public information.
10. Wage Rate Determination discussion
Offerors should discuss understanding that the Service Contract Act will be applicable in any resultant base contract and the Wage Rate determination for the Offeror’s locality will be incorporated in the base contract. The Offeror should list the direct labor positions proposed to conduct Sample Task Orders A and B in this section and acknowledge understanding that the minimum wage rates will comply with the relevant locality wage determination. Refer to Article I.6 SERVICE CONTRACT LABOR STANDARDS of the Solicitation. The Offeror may also refer to https://www.wdol.gov/sca.aspx for additional information.
11. Small Business Subcontracting Plan
a. Refer to Section L.2.c.6. Small Business Subcontracting Plan for details and use Attachment 16 in Section J of the Solicitation to complete the plan.
b. The plan must be completed based upon the IDIQ Maximum which is $500,000,000 and should be listed as the total contract amount in the plan.

c. The percentages proposed must be based on the total IDIQ base value of $500,000,000, not the proposed cost of the Sample Task Orders. In other words, the Small business subcontracting Plan percentage goals need to be based on your estimated amount you plan to subcontract over the 10-year period of performance of the IDIQ based upon the assumption that the Offeror would receive $500M in task order awards. All goals must be estimated and realistic percentages must be calculated against the total IDIQ base value of $500M
d. Additionally, all goals must meet the minimum HHS current subcontracting goals; these percentages are located on pages 2 and 3 of the Small Business Subcontracting Plan. The HHS current subcontracting goals are listed in L.2.c.6.
e. The Total estimated dollar value of ALL planned subcontracting (2.a.) in the plan must be determined by dividing the Ceiling/Maximum of $500M by the total costs proposed for all years for both Sample Task Orders A and B to arrive at a factor (xx.xx). This factor should then be multiplied by Total Other Direct Costs ($xxx,xxx) of both Sample Task Order A and B for all years to arrive at the Total estimated dollar value of ALL planned subcontracting.
f. All costs proposed in the budget (other than the Prime Offeror’s labor) must be considered for small business subcontracting opportunities and the Offeror must provide detailed justification as to why these opportunities cannot be fulfilled by a small business, if not proposed as such.
12. Subcontract Documentation
a. The signed subcontractor commitment letters must be submitted as part of the Technical Proposal and will be evaluated by the Technical Committee. Reference the location of the commitment letters in the business proposal. The remainder of the subcontract documentation must be submitted in the business proposal.
b. As per FAR 44.1 – General, “Subcontract” means any contract as defined in Subpart
2.1 entered into by a subcontractor to furnish supplies or services for performance of a prime contract or a subcontract. It includes but is not limited to purchase orders, and changes and modifications to purchase orders.
c. The Prime must provide the following for each proposed subcontractor:
i. Discussion of determination of responsibility (Refer to FAR 9.104-4).
ii. Confirmation of completion of electronic annual representations and certifications at www.sam.gov
iii. Discussion of how price or cost reasonableness was determined including discussion of adequate price competition or its absence properly justified
iv. Discussion of price/cost analysis of the subcontractor’s cost/price proposal
v. For cost-type subcontracts provide an unsigned Draft Subcontract Agreement;
Use of the Offeror’s standard template including the subcontractor organization’s name, address, type of subcontract, period of performance, amount of subcontract, relevant flowdown clauses, and the subcontractor’s statement of work.
