About this file

This document is a certification form required as part of a solicitation response for maintenance, inspection, and testing services for an electrical power distribution system. The solicitation was issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 1. The certification requires the offeror to attest whether subcontractors will be used, and if so, to provide details including business names, DUNS numbers, descriptions of work, and the percentage of contract cost for the prime contractor and any similarly situated subcontractors. It warns that penalties may be assessed if the limitations on subcontracting requirements in 13 C.F.R. 125.6 are violated during contract performance. The contracting officer may require demonstration of compliance with these limitations at any time.
