About this file
This document is a Minority Business Enterprise (MBE) Form from the City of Fort Myers, implementing Resolution 38-125, which establishes a 15% MBE goal for city contracts. The form is designed to document contractors' good faith efforts in meeting minority business participation requirements, with sections for prime and subcontractor information, record of transmittal letters, telephone/in-person contact documentation, handling of incomplete quotations, and reasons for non-utilized MBE firms.
The form provides a structured approach for contractors to demonstrate their commitment to minority business inclusion, requiring detailed documentation of outreach efforts, communication methods, and rationale for MBE selection or non-selection. Contractors must complete steps including providing firm contact information, describing work to be performed, maintaining records of communication attempts, and explaining any instances where minority business enterprises are not ultimately utilized in the project. The goal is to ensure transparent and equitable procurement processes that support minority business participation in city contracts.
