About this file
This document is a Build America, Buy America (BABA) Certification Form issued by the City of Fort Myers Procurement Services for a Parks & Rec Pavilion Replacements project. The form requires contractors to certify compliance with federal requirements mandating the use of iron, steel, manufactured products, and construction materials produced in the United States, as stipulated by the Infrastructure Investment and Jobs Act passed on November 15, 2021. The certification applies to all project portions, including subcontracted work, and requires contractors to identify domestic sources of BABA-covered products and provide necessary verification documentation.
The certification form obligates contractors to affirm their understanding and commitment to BABA provisions, with a clear warning that false statements may result in rejection or termination of awards or contracts. The form includes fields for contractor details such as signature, name, title, company information, contact details, and provides a link to the White House website for additional information about BABA requirements and authorization. This certification is specifically tied to the City of Fort Myers' project involving prefabricated pavilion replacements at four different park locations: Dunbar Park, Jefferson Park, Dupree/Aztec Park, and Freemont Park.
