TREATMENT ROOM SHEILDING REQUIREMENTS

WARNING

Varian Medical Systems shall have no approval or other responsibility for any matter affecting or related to the adequacy of the radiation protection walls and barriers or related safety devices. All radiation shielding designs must meet codes and regulations of all Authorities Having Jurisdiction (AHJ) and must be approved by the Customer’s or Facility’s Physicist of Record and shall be the sole responsibility of the Customer/Facility. The hours of operation, patient workload, accelerator energy, and the shielding materials should all be taken into consideration when calculating shielding requirements. Severe injury or death can result from improper radiation shielding.

WARNING

A qualified physicist needs to be available to complete a preliminary radiation survey per Varian Rad safety instruction L9330 (includes links to L9330A form for AMS & to L9205 & L9206 for APAC & EMEA). The survey needs to take place at initial beam on, approximately 5 days after the accelerator is rigged in and installation started. Failure to perform this survey or unsatisfactory results will stop the installation. The radiation survey form will be provided by the Varian Project Manager and/or the Varian Installation Team.

The Halcyon and Ethos systems only produce a 6 MV FFF beam. Due to the transmission factor of the integrated beam-block being equivalent to the head leakage value, a primary shielding wall is not required for the room. Therefore, secondary shielding walls will typically satisfy radiation shielding requirements.

Refer to the following documents for additional information:

• Halcyon and Ethos Radiotherapy System IEC Accompanying Documents Type Tests and Procedures

• Ethos Product Spec Sheet RAD10674

• Halcyon Product Spec Sheet RAD10506

For documentation contact your Varian Sales District Manager or Varian Regional Planner documents, or go to www.MyVarian.com.

Magnetic Field Shielding Considerations

• Consideration should be taken when locating linear accelerator equipment in the proximity of Magnetic Resonance Imaging (MRI) units or other magnetic field generating equipment.

• The Halcyon Accelerator and associated video monitors should be located outside of the 100 μT (1 Gauss) magnetic field.

Radiation Shielding Details

• Treatment room shielding is required for the protection of therapists and others while the Halcyon/Ethos accelerator has the beam on. Accelerator shielding is provided by either poured-in-place concrete, lead/steel plates, or a combination.

• The amount and type of shielding on treatment room entrance doors have varying requirements based on the presence and length of the maze, and the energy of the Accelerator. The exact composition of the door shielding must be designed and approved by the Physicist of Record.

Typically, shielded doors must be “exempted” where fire code labeling is required.

• Most treatment rooms are entered through a maze. This hallway is designed to reduce radiation levels at the entrance door. The length of the maze and the occupancy outside the entrance door affects the amount of shielding required in the door. “No-maze” doors are available from several shielding manufacturers. The use of these doors must be reviewed by the Physicist of Record early in the design process.

• To reduce radiation exposure outside room, air handling ducts should enter/exit the room through penetration(s) above the maze door. The ducts should be placed as high as possible in order to minimize radiation exposure to occupied space. The ducts should be designed to minimize the area of penetration through the wall. In most cases, duct shielding will not be required. Clear space should be left around the duct (outside the treatment room) for shielding retrofit, in case the post installation radiation survey indicates a requirement. Penetration, including ducts, directly into the treatment room should be avoided. For no-maze treatment rooms, duct design and shielding must be addressed by the Physicist of Record.

• Provide adequate radiation shielding (usually lead or steel with a 25 mm [1"] margin) behind all junction and pull boxes recessed in concrete walls. Verify thickness and location with the Physicist of Record. Steel or lead shielding at Accelerator rooms may be embedded in or mounted on the inside surface of concrete walls and ceiling. Additional structural reinforcement may be required.

• Except where specifically noted, radiation leakage in non-controlled areas shall not exceed 20 µSv/week (2 mrem/week), assuming 100% occupancy beyond the shielding barriers (per NCRP Report 91 “Recommendations of Limits for Exposure to Ionizing Radiation”).

• Supplement No. 11 (1972) and Supplement No. 17 (1983) of the “British Journal of Radiology” describes two different conventions for referencing the quality of an x-ray radiotherapy beam. The convention presented in Supplement No. 11 has been adopted for this sheet.

• The Physicist of Record for the project should become involved with the treatment room design as early as possible. Regarding facility shielding, the Physicist of Record is responsible for designing the treatment room radiation shield barriers and confirming they meet applicable regulatory requirements.

The facility design is based on regulatory requirements of the regulatory body tasked with oversight of Radiation Producing Devices in the Region, and recommendations of the National Council of Radiation Protection and Measurement (NCRP).

Att. Tim Bleck

RE: Shielding Calculations for:

Halcyon Vault

Brooklyn VA Medical Center

800 Poly Place

Brooklyn, NY 11209

Dear Mr. Bleck:

Enclosed are shielding calculations for the Varian Halcyon 6MV Linear Accelerator located at the above referenced facility. The following pages describe the shielding design in detail using the principles found in NCRP Report No. 151, Shielding Techniques for Radiation Oncology

Facilities, by Patton H. McGinley, the Varian site planning guide, and supplied diagrams.

Page 2 of this report describes the assumptions used for the calculations. These are based upon information provided including structural drawings, estimated workloads, and unit specifications.

Please ensure all assumptions are reviewed by the medical physics/clinical staff for accuracy and notify our office immediately should any changes occur.

Page 3 of this report describes the shielding plan in detail. Please reference the "Install" column for shielding required as well as any noted comments. Wall shielding should be installed to the shielded ceiling above. The shielding needed is based upon noted material densities. Should the building material change, please contact our office immediately to ensure shielding adequacy.

A summary of our analysis is as follows:

1. Add 3" of lead (Pb.) to the maze endcap from floor to vault ceiling as shown on diagram.

2. Add 3" of lead (Pb.) to wall 6 from floor to vault ceiling as shown on diagram.

3. Ensure the existing 4" Pb. installed on the remaining maze wall is not removed.

4. Ensure the vault door has a minimum of 1/4" Pb. shielding installed.

Please do not hesitate to contact our office should you need additional information.
