About this file

This is an Annual Procurement Disclosure Statement (SPO Form 110) completed by Jessica Payne, High Risk Perinatal Program Manager at the Arizona Department of Health Services. The form is effective for fiscal year 2026 and was signed on June 20, 2025. The document is a conflict of interest and procurement ethics disclosure required under Arizona Revised Statutes (A.R.S.) §§ 38-501 to 38-511, 41-741, 41-753, 41-2503, 41-2517, and 41-2616 for state employees with significant procurement roles.

The disclosure statement certifies that Jessica Payne has no substantial pecuniary or proprietary interests (marked as "N/A") that would create conflicts of interest with state contracts. She attests to compliance with ethical procurement standards, including commitments to maintain confidentiality, avoid providing unfair advantages to vendors, base solicitation development solely on state requirements, and disclose any conflicts of interest that may arise. The form establishes that these obligations remain in effect following any termination of employment and apply to any subsequent employment with entities that may respond to state solicitations, with a one-year restriction on employment discussions with vendors involved in active procurements.
