About this file
This document contains a past performance questionnaire and rating guidelines for the Personnel and Readiness Infrastructure Support Program (PRISM) Multiple Award Task Order Contract (MATOC). The past performance questionnaire requests information on a contractor's previous contracts including contract details, value, period of performance, description of work, complexity, ratings for quality, cost control, customer satisfaction, timeliness and business relations. The rating guidelines define adjectival ratings for contractor performance in these areas from unsatisfactory to exceptional. The related federal contract opportunity notice solicits proposals for the PRISM MATOC to provide enterprise solutions and services to the OUSD Personnel & Readiness, its Directorates and Strategic Mission Partners. Offerors shall utilize templates provided to submit questions by February 14, 2024 and full proposals by March 6, 2024. The MATOC will have separate pools for Personnel and Readiness work and Major DoD Systems work, utilizing contract types including firm-fixed-price, cost-plus-fixed-fee, and hybrid task orders.
