About this file
This is an Insurance Checklist document issued by the City of West Sacramento, California, detailing certificate of insurance requirements for standard city contracts with project values between $2 million and $5 million. The document specifies mandatory insurance coverage requirements that all contractors and vendors must obtain and maintain throughout the contract term. Required coverage includes Commercial General Liability at $2,000,000 per occurrence/$5,000,000 aggregate, Commercial Automobile Liability at $1,000,000 per accident, Workers' Compensation per statutory requirements, and Professional Liability (Errors & Omissions) at $2,000,000 per claim if applicable. The City of West Sacramento, with offices located at 1110 West Capitol Avenue, West Sacramento, CA 95691, must be named as an additional insured on all policies except Workers' Compensation and Professional Liability. All certificates of insurance must provide thirty days' written notice to the Certificate Holder prior to any cancellation or material change of coverage.
All General Liability and Automobile Liability policies must include Additional Insured Endorsements equivalent to CG 2010 0714, Primary and Noncontributory Endorsements, and Separation of Insureds endorsements naming the City, its officers, officials, employees, agents, and volunteers as additional insureds. Workers' Compensation policies must include a Waiver of Subrogation Endorsement in favor of the City and its representatives, or a Workers' Compensation Waiver if the contractor is self-employed with no employees. Contractors and vendors are responsible for forwarding this insurance checklist to their insurance agent or broker, and certificates will be considered incomplete if the Additional Insured Endorsement is not attached verifying that the required endorsements have been executed. This checklist applies to the Washington District Infrastructure Upgrade Project, a comprehensive municipal infrastructure improvement initiative involving water main installation, sewer rehabilitation, and fiber optic conduit installation with a total project budget of $5 million.
