The form includes warning statements about penalties for false or misleading information (18 U.S.C. 1001) and false claims (31 U.S.C. 3729-3731 and 18 U.S.C. 287). This specific form is associated with solicitation 140A1125Q0030 for SCIP-IW Building Transfer ESA/Lead/ASB under the Department of the Interior Bureau of Indian Affairs Western Region.
