PAST PERFORMANCE QUESTIONNAIRE
REQUEST FOR PROPOSALS FOR
MAINTENANCE AND DEACTIVATION OF
TEXAS TEMPORARY HOUSING UNITS

Name of contractor requesting the reference:
The purpose of this questionnaire is to obtain information regarding the quality of the above-named Contractor’s past performance relative to a contract, completed or in progress, at your company/agency. The Federal Emergency Management Agency is considering this Contractor for maintenance and deactivation of temporary housing units which include mobile homes, prefabricated units, and travel trailers. We would greatly appreciate your input regarding the quality of the Contractor’s past performance.
Handwritten responses are sufficient. It is requested that the individual(s) responsible for the administrative oversight of the project (Contracting Officer, Program Manager, or COTR) respond to this questionnaire. More than one copy of the questionnaire may be submitted if responses from more than one individual are appropriate.

1. How consistent is the Contractor in providing services?

0 = Services are frequently lacking, late and/or incomplete.
10 = Services are consistently provided in a timely manner.

2. How consistent is the Contractor in providing required information (forms, reports, publications -- both electronic and hard copy) in a timely manner, error free and grammatically correct? Where:
0 = Contractor employees consistently do not provide all documentation in a timely manner, error free and grammatically correct.
10 = Contractor employees consistently provide all documentation in a timely manner, error free and grammatically correct.

3. How consistent is the Contractor in keeping records and related documentation complete, accurate, and current (both electronic and hard copy)? Where:
0 = Documentation is often missing, inaccurate, or out of date.
10 = Documentation is consistently on file, accurate, and current.

4. Rate the overall level of professionalism of the Contractor and his/her workforce in carrying out these duties? Where:
0 = Contractor employees demonstrate a low level of understanding of their assignments, are often engaged in unauthorized activity, and are inattentive to their duties.
10 = Contractor employees consistently understand their assignments, carry out their jobs, and are fully attentive to their duties.

5. How effective is the Contractor in identifying potential problems and resolving them on his/her own? Where:
0 = Contractor consistently fails to identify and resolve problems on his own.
10 = Contractor is consistently pro-active in identifying potential problems and recommending solutions.

6. How effectively does the Contractor respond to problems presented to him/her by your agency or business? Where:
0 = Contractor consistently fails to react and to successfully address problems presented to him by agency or business representative.
10 = Contractor consistently reacts appropriately and successfully addresses problems presented to him by agency or business representative in a timely manner.

7. Rate the Contractor’s quality control of the project.
0 = Contractor does not resolve quality of work issues, and the project suffers due to lack of quality control.
10 = Contractor resolves quality of work issues and services quickly and work flows smoothly, resulting in a timely high quality project.

0 = Contractor is not proactive, and services suffer due to lack of management.
10 = Services progress smoothly, management is proactive and responsive, and the project is successful.
